内镜下Olympus金属钛夹与和谐夹对消化性溃疡活动性出血的效果比较

摘　　要：目的 观察在腹腔镜下Olympus金属钛夹与和谐夹对消化性溃疡(PU)活动性出血患者的应用效果。方法 选取濮阳市人民医院2020年4月至2021年10月收治的180例PU活动性出血患者为对象,随机分为对照组与观察组,各90例。所有患者均接受内镜下治疗,对照组采用Olympus金属钛夹,观察组接受和谐夹治疗。比较两组输血量、住院时间、止血夹使用数量、止血有效率、即时止血率、再出血率,比较两组治疗前后血液指标及治疗期间不良反应发生情况。结果 观察组输血量、住院时间、止血夹使用数量低于对照组;观察组止血有效率(97.78%)高于对照组(90.00%),差异有统计学意义(P<0.05)。两组即时止血率比较,差异无统计学意义(P> 0.05)。观察组再出血率低于对照组,差异有统计学意义(P<0.05)。治疗后1周,观察组血红蛋白、血细胞比容水平显著高于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率(1.11%)低于对照组(4.44%),差异无统计学意义(P> 0.05)。结论 对PU活动性出血患者分别以和谐夹、Olympus金属钛夹进行止血治疗,和谐夹较金属钛夹止血效果更好,可降低再出血率,安全性佳,值得临床使用。Objective To observe the application effect of laparoscopic Olympus titanium clip and harmony clip in patients with active peptic ulcer(PU).Methods A total of 180 patients with PU active bleeding admitted to the Puyang City People's Hospital from April 2020 to October 2021 were selected as the subjects,and they were divided into control group and observation group according to the random number table method,with 90 cases in each group.All patients received endoscopic treatment,the control group received Olympus metal titanium clips,and the observation group received harmonious clips.Blood transfusion volume,hospital stay,number of hemostatic clips,effective hemostasis rate,immediate hemostasis rate,and rebleeding rate were compared between the two groups.Comparison of blood indexes before and after treatment and the occurrence of adverse reactions during treatment between the two groups.Results Comparison of blood transfusion volume,hospital stay,and number of hemostatic clips used in the observation group was lower than that in the control group.The effective rate of hemostasis in the observation group was 97.78%,which was higher than 90.00% in the control group,with significant difference(P<0.05).There was no statistical difference in the immediate hemostasis rate between the two groups(P>0.05),and the rebleeding rate was compared,and the observation group was lower than the control group,with significant difference(P<0.05).One week after treatment,hemoglobin and hematokrit in the observation group were significantly higher than those in the control group,with significant differences(P<0.05).The incidence of adverse reactions in the observation group(1.11%) was lower than that in the control group(4.44%),with no statistically significant difference(P>0.05).Conclusion For the patients with PU active bleeding,harmony clamp and Olympus metal clamp are used respectively.Harmony clamp has better hemostatic effect than metal clamp,which can reduce the rebleeding rate,has good safety,and is worthy of clinical use.
