参芪归血汤配方颗粒联合化疗对SQBD型晚期NSCLC患者血清GM-CSF、G-CSF及免疫指标的影响

摘　　要：目的研究参芪归血汤配方颗粒联合化疗对气血亏虚证(SQBD)型晚期NSCLC患者血清GM-CSF、G-CSF及免疫指标水平的影响。方法依据治疗方案,随机选出2020年7月至2021年12月符合纳入标准且仅给予常规NP化疗方案的SQBD型晚期NSCLC患者42例为对照组,选出同期给予相同化疗方案并加服参芪归血汤配方颗粒的42例为观察组。比较两组外周血细胞参数水平、免疫指标参数水平、肿瘤标志物水平、血清GM-CSF和G-CSF水平,KPS评分、不良反应发生率和化疗疗效。结果观察组WBC、HGB、PLT和Neu参数水平均显著高于对照组(第8、22、36天组间比较)(P<0.05);研究后,观察组免疫指标参数水平、肿瘤标志物水平降低幅度、血清GM-CSF和G-CSF水平、KPS评分及其改稳率等方面显著优于对照组(P<0.05),观察组WBC降低、HGB降低、Neu减少、恶心呕吐和便秘等不良反应发生率较对照组明显减少(P<0.05);观察组化疗总有效率62.50%显著大于对照组39.02%(P<0.05)。结论联用参芪归血汤配方颗粒的给药方案对SQBD型晚期NSCLC患者化疗更具优势,能从补气、养血两个方面,改善患者SQBD症状,对提升患者生活质量、降低部分不良反应发生率、提高临床化疗疗效有帮助,其机制与参芪归血汤配方颗粒能有效调节患者血清中GM-CSF、G-CSF及免疫指标水平有关。Objective The purpose of this study is to investigate the effect of Shenqi Guixue Decoction formula granules combined with chemotherapy on GM-CSF、G-CSF and immune indexes in serum in patients with advanced NSCLC with SQBD type.Methods From July 2020 to December 2021 depending on the treatment regimen,42 cases that met the inclusion criteria,advanced NSCLC with SQBD type,who were routinely given NP(vinorelbine plus cisplatin)regimen were selected as the control group.The other 42 cases who were given the same chemotherapy regimen and taken additional Shenqi Guixue Decoction formula granules were selected as the observation group in the same period.In both groups,the parameter levels of peripheral blood cells,the levels of GM-CSF and G-CSF in serum,the immune indexes'levels,the tumor markers'levels,KPS score,the adverse reaction rates and the chemotherapy efficacy were analyzed.Results At 8th,22nd and 36th,the parameter levels of WBC,HGB,PLT and Neu in the observation group were noteworthily higher than those in the control group(P<0.05).The immune indexes'levels,the decline of tumor markers'levels,the GM-CSF and G-CSF levels in serum,KPS score and its improvementstability rate in the observation group were superior to the control group after the study(P<0.05).Furthermore,compared with the control group,nauseaand-vomiting,constipation,leucopenia,reduction in hemoglobin and neutropenia's adverse reaction rates in the observation group experienced a significant decrease after the study(P<0.05).The total effective rate of chemotherapy 62.50%in the observation group was significantly higher than that 39.02%in the control group(P<0.05).Conclusion The combined dosage regimen of Shenqi Guixue Decoction formula granules has more advantages in the chemotherapy treatment of patients with advanced NSCLC with SQBD type.It can improve patients'SQBD symptoms from two aspects of tonifying qi and nourishing blood.It is helpful to improve the quality of life,reduce some adverse reaction rates and improve the efficacy of clinical
