摘  要:目的:探究宫颈病变筛查中开展DNA倍体分析的作用。方法:于2019年1月至2021年9月选择本院接收的宫颈筛查女性共计1713例作为研究对象。研究对象全部开展宫颈活检以及宫颈细胞DNA倍体分析,并将病理检测结果作为判断的主要依据。观察病理检测结果;观察DNA倍体分析异倍体数目;观察宫颈DNA倍体分析的敏感度;观察DNA倍体分析DNA指数情况;观察DNA倍体分析DNA指数的敏感度。结果:1713例女性在实施病理组织检查后,结果提示正常者有821例(47.93%),异常者有892例(52.07%)。在892例异常者中,低级别鳞状上皮内病变者有659例(38.47%)、高级别鳞状上皮内病变者有196例(11.44%)、宫颈癌者有37例(2.16%)。实施宫颈细胞DNA倍体分析异常体数目后,结果提示少量DNA倍体者有677例(39.52%),中量DNA倍体者有705例(41.16%),大量DNA倍体者有331例(19.32%)。少量DNA倍体异常、中量DNA倍体异常、大量DNA倍体异常诊断敏感度依次是47.08%、46.24%、76.13%。在高级别鳞状上皮内病变、宫颈癌方面,大量DNA倍体异常诊断敏感度均高于少量DNA倍体异常、中量DNA倍体异常(P<0.05)。在实施宫颈细胞DNA倍体分析DNA指数判断后,结果提示758例(44.25%)属于2.5<DNA指数<3.5,955例(55.75%)属于DNA指数>3.5。2.5<DNA指数<3.5异常、DNA指数>3.5异常诊断敏感度依次是37.47%、63.77%。在高级别鳞状上皮内病变、宫颈癌方面,DNA指数>3.5诊断敏感度均高于2.5<DNA指数<3.5(P<0.05)。结论:DNA检测具有快速、精准、客观的应用优点,具有较高的敏感性,可发现宫颈病变并预测其病变程度,值得在宫颈病变筛查中推广。Objective:To analyze the effects of DNA ploidy analysis on the screening of cervical lesions.Methods:A total of 1713 women who underwent cervical screening at our hospital were selected as the study subjects from January 2019 to September 2021.The cervical biopsy and DNA ploidy analysis were performed.The pathological examination results were used as the main basis for diagnosis.The pathological examination results,the number of aneuploidies in DNA ploidy analysis,the sensitivity of cervical DNA ploidy analysis,the sensitivity of DNA ploidy analysis DNA index and DNA ploidy analysis DNA index were analyzed.Results:The pathological examination results indicated 821 normal cases(47.93%)and 892 abnormal cases(52.07%).Among 892 abnormal cases,659 patients were found with low-grade squamous intraepithelial lesion(38.47%),196 cases of high-grade squamous intraepithelial lesion(11.44%)and 37 cases of cervical cancer(2.16%).DNA ploidy analysis results indicated 677 cases with a small amount of DNA aneuploid(39.52%),705 cases with a moderate amount of DNA aneuploid(41.16%)and 331 cases with a large amount of DNA aneuploid(19.32%).The diagnosis sensitivity based on a small,moderate and large amount of DNA aneuploid was 47.08%,46.24%and 76.13%.The diagnosis sensitivity of high-grade squamous intraepithelial lesion and cervical cancer based on a large amount of DNA aneuploid was higher than that based on a small and moderate amount of DNA aneuploid(P<0.05);DNA index results showed that 758 patients(44.25%)had 2.5<DNA index<3.5 and 955 patients(55.75%)with DNA index>3.5.The diagnosis sensitivity of DNA aneuploid based on 2.5<DNA index<3.5 and DNA index>3.5 was 37.47%and 63.77%;the diagnosis sensitivity of high-grade squamous intraepithelial lesion and cervical cancer based on DNA index>3.5 was higher than that based on 2.5<DNA index<3.5(P<0.05).Conclusion:DNA is featured as quickness,accuracy,objectiveness and high diagnosis sensitivity,providing basis on the screening of cervical lesions and the prediction of the disease sev
