摘 要:【目的】探讨氟代脱氧葡萄糖(^(18)F-FDG)正电子发射断层扫描(PET)-CT显像代谢参数及血清白蛋白(ALB)、前白蛋白(PA)水平与食管癌患者术后预后的相关性。【方法】食管癌手术患者90例,按照预后情况分为预后良好组(30例)、预后不良组(60例),收集并比较两组患者一般资料、^(18)F-FDG PET-CT显像代谢参数、血清营养指标(ALB、PA);采用受试者工作特征(ROC)曲线分析^(18)F-FDG PET-CT显像代谢参数及ALB、PA预测食管癌术后患者预后的价值,采用多因素Logistic回归分析食管癌患者术后预后不良的危险因素。【结果】两组年龄、性别、吸烟史、饮酒史、身高、体重、家族史比较,差异均无统计学意义(P>0.05);预后不良组最大标准摄取值(SUV_(max))、代谢体积(MTV)以及病灶糖酵解总量(TLG)均高于预后良好组(P<0.05);预后不良组ALB、PA水平均低于预后良好组(P<0.05);经ROC曲线分析显示,SUV_(max)、MTV、TLG、ALB、PA均可用于食管癌患者术后预后不良的预测,曲线下面积分别为0.985、0.913、0.963、0.726、0.731(均P<0.05);经多因素Logistic回归分析证实,SUV_(max)≥21.25、MTV≥14.42 cm 3、TLG≥5.85、ALB<52.795 g/L、PA<230.09 mg/L是食管癌患者术后预后不良的危险因素(P<0.05)。【结论】^(18)F-FDG PET-CT显像代谢参数及ALB、PA水平能预测食管癌手术患者的预后,临床可定时监测上述指标以及时跟踪术后化疗,改善患者预后。【Objective】To explore the correlation of postoperative prognosis in patients with esophageal cancer with fluorodeoxyglucose(^(18)F-FDG)positron emission tomography(PET)-CT metabolic parameters and levels of serum albumin(ALB)and prealbumin(PA).【Methods】Ninety patients with esophageal cancer were divided into two groups according to prognosis:30 patients with good prognosis and 60 patients with poor prognosis.General information,^(18)F-FDG PET-CT metabolic parameters,serum nutritional indicators(ALB and PA)of both groups were collected and compared.Receiver operating characteristic curve(ROC)analysis was used to evaluate the value of ^(18)F-FDG PET-CT metabolic parameters,ALB,and PA in predicting postoperative prognosis in patients with esophageal cancer,and multiple logistic regression analysis was used to identify the risk factors for poor postoperative prognosis.【Results】There were no significant differences in age,sex,smoking history,drinking history,height,weight,and family history between the two groups(P>0.05).The maximum standard uptake value(SUV_(max)),metabolic volume(MTV),and total lesion glycolysis(TLG)in the poor prognosis group were higher than those in the good prognosis group(P<0.05).The levels of ALB and PA in the poor prognosis group were lower than those in the good prognosis group(P<0.05).ROC analysis showed that SUV_(max),MTV,TLG,ALB,PA could be used to predict poor postoperative prognosis in patients with esophageal cancer,with the areas under the curve being 0.985,0.913,0.963,0.726 and 0.731 respectively(all P<0.05).Multivariate logistic regression analysis confirmed that SUV_(max)≥21.25,MTV≥14.42 cm^(3),TLG≥5.85,ALB<52.795 g/L,and PA<230.09 mg/L were risk factors for poor postoperative prognosis in patients with esophageal cancer(P<0.05).【Conclusion】The ^(18)F-FDG PET-CT metabolic parameters and serum levels of ALB and PA can be used for predicting the prognosis of surgical patients with esophageal cancer.Clinicians can regularly monitor these indicators and promptly c
