摘  要:【目的】探讨基于生物力学原理下的康复训练联合视觉反馈平衡训练对原发性帕金森病(PD)患者运动功能的影响。【方法】延安大学咸阳医院收治的120例PD患者,随机分为观察和对照组,每组60例。对照组患者实施基于生物力学原理的康复训练,观察组在此基础上联合实施视觉反馈平衡训练。于训练前、训练3个月后,比较两组患者的肢体运动能力、躯干控制能力、步行稳定性。【结果】训练前,两组患者肢体运动功能、躯体控制能力、Berg平衡量表(BBS)和步态指数(DGI)评分比较,差异均无统计学意义(P>0.05);训练3个月后,观察组患者的下肢、上肢等肢体功能总评分高于对照组(P<0.05);观察组患者在床上向健侧转、在床上向患侧转、从卧位坐起、无扶手椅子上坐立保持平衡等躯干控制能力评分高于对照组(P<0.05);观察组BBS、DGI评分均高于对照组(P<0.05)。【结论】基于生物力学原理的康复训练联合视觉反馈平衡训练可有效提高PD患者的肢体运动能力,加强躯干控制能力,维持其机体平衡与步行稳定性。【Objective】To investigate the impact of rehabilitation training based on biomechanical principles combined with visual feedback balance training on motor function in patients with idiopathic Parkinson's disease(PD).【Methods】A total of 120 PD patients admitted to Yan'an Hospital of Xi'an Medical University were randomly divided into two groups,with 60 patients in each group.The control group received rehabilitation training based on biomechanical principles,while the observation group received additional visual feedback balance training on top of the basic rehabilitation training.The limb motor function,trunk control ability,and walking stability of the patients were compared between the two groups before and after 3 months of training.【Results】Before training,there were no statistically significant differences in limb motor function,trunk control ability,Berg Balance Scale(BBS)scores,and Dynamic Gait Index(DGI)scores between the two groups(P>0.05).After 3 months of training,the total scores of limb function in the observation group were higher than those in the control group(P<0.05).The trunk control ability scores in the observation group(in aspect of turning to the healthy side,turning to the affected side on the bed,sitting up from a lying position,and maintaining balance on an armless chair)were higher than those in the control group(P<0.05).The BBS and DGI scores in the observation group were also higher than those in the control group(P<0.05).【Conclusion】Biomechanical principles-based rehabilitation training combined with visual feedback balance training can effectively improve limb motor function,enhance trunk control ability,and maintain body balance and walking stability in patients with PD.
