摘　　要：目的分析口腔天疱疮患者血清抗桥粒芯蛋白1(Dsg1)、抗桥粒芯蛋白3(Dsg3)及大疱性类天疱疮抗原2(BP180)的表达水平及意义。方法抽取2020年1月至2022年12月南阳市口腔医院收治的86例疑似口腔天疱疮患者作为观察组,并以1∶1配比抽取同期排除天疱疮疾病的86例健康人作为对照组。所有研究对象均检测血清Dsg1、Dsg3、BP180水平,统计观察组阳性率;比较观察组阳性患者和对照组血清Dsg1、Dsg3、BP180抗体水平;比较观察组阳性患者中不同病情严重程度、不同病程者血清Dsg1、Dsg3、BP180抗体水平。结果观察组86例疑似口腔天疱疮患者中,确诊阳性46例,阳性率为53.50%(46/86),其中Dsg1阳性15例(2.60%,15/46),Dsg3阳性26例(56.50%,26/46);BP180阳性17例(37.00%,17/46)。观察组阳性患者血清Dsg1、Dsg3、BP180抗体水平高于对照组(P<0.05)。观察组阳性患者中不同病情程度者血清Dsg1、Dsg3、BP180抗体水平比较差异有统计学意义(P<0.05);随病情加重,血清Dsg1、Dsg3、BP180抗体水平呈增高趋势,且重度、中度患者抗体水平高于轻度患者(P<0.05)。观察组阳性患者中不同病程者血清Dsg1、Dsg3、BP180抗体水平比较差异有统计学意义(P<0.05);随病程延长,血清Dsg1、Dsg3、BP180抗体水平呈降低趋势,且随访1、3个月时抗体水平低于初发时(P<0.05)。结论口腔天疱疮患者血清Dsg1、Dsg3、BP180抗体表达水平较高,并与病情严重程度、病程进展密切相关。Objective To analyze the expression and significance of serum desmoglein 1(Dsg1),desmoglein 3(Dsg3)and type XⅦcollagen(BP180)in patients with oral pemphigus.Methods A total of 86 patients suspected of oral pemphigus who were treated in Nanyang Stomatological Hospital from January 2020 to December 2022 were selected as the observation group,and 86 healthy people excluded from pemphigus during the same period were selected as the control group with a 1∶1 ratio.The levels of serum Dsg1,Dsg3 and BP180 in all selected patients were detected,and the positive rate of the observation group was statistically observed.The levels of serum Dsg1,Dsg3 and BP180 antibodies were compared between positive patients in the observation group and patients in the control group.The levels of serum Dsg1,Dsg3 and BP180 antibodies in positive patients with different disease severity and different course of disease were compared in the observation group.Results Among the 86 patients suspected of oral pemphigus in the observation group,46 cases were confirmed positive,with a positive rate of 53.50%(46/86),including 15 cases Dsg1 positive(2.60%,15/46),26 cases Dsg3 positive(56.50%,26/46)and 17 cases BP180 positive(37.00%,17/46).The levels of serum Dsg1,Dsg3 and BP180 antibodies in positive patients in the observation group were higher than those in the control group(P<0.05).There were statistically significant differences in levels of Dsg1,Dsg3 and BP180 antibodies among positive patients with different disease degrees in the observation group(P<0.05).The levels of Dsg1,Dsg3 and BP180 antibodies showed an increasing trend with the aggravation of the disease,and the levels of antibodies in severe and moderate patients were significantly higher than those in mild patients(P<0.05).The difference in levels of Dsg1,Dsg3 and BP180 antibody was statistically significant among positive patients with different severity degrees in the observation group(P<0.05).The levels of Dsg1,Dsg3 and BP180 antibodies showed a decreasing trend with the prolonga
