摘　　要：目的研究外周血神经丝轻链(NFL)、钙结合蛋白S100β(S100β)水平与脑出血患者血脑屏障(BBB)指数及认知功能的关系。方法将2019年7月至2022年6月郑州大学第五附属医院收治的92例脑出血患者纳为脑出血组,另选取同期92例体检正常者纳为对照组,比较两组血清NFL、S100β水平及BBB指数、简易智力状态检查量表(MMSE)评分,比较BBB指数不同的脑出血患者间血清NFL、S100β水平,比较MMSE评分不同的脑出血患者间血清NFL、S100β水平,采用Pearson检验分析脑出血患者血清NFL、S100β水平与BBB指数、MMSE评分之间的相关性。结果与对照组比较,脑出血组血清NFL、S100β水平及BBB指数明显增高(P<0.05),MMSE评分明显降低(P<0.05);与BBB指数<0.38患者比较,BBB指数≥0.38患者血清NFL、S100β水平明显增高(P<0.05);与MMSE评分≥23.36患者比较,MMSE评分<23.36患者血清NFL、S100β水平明显增高(P<0.05);相关性检验结果提示,脑出血患者血清NFL、S100β水平与BBB指数呈正相关(P<0.05),与MMSE评分呈负相关(P<0.05)。结论外周血NFL、S100β在脑出血患者中水平较高,两者与血脑屏障指数、认知功能损害之间均有密切关联,其水平检测有助于临床早期发现脑出血患者脑组织损伤程度、认知障碍并预测疾病进展。Objective To study the correlation of peripheral blood neurofilament light chain(NFL)and S100 calcium-binding proteinβ(S100β)levels with blood brain barrier(BBB)index and cognitive function in patients with intracerebral hemorrhage.Methods Ninety-two patients with intracerebral hemorrhage in the Fifth Affiliated Hospital of Zhengzhou University from July 2019 to June 2022 were enrolled as the hemorrhage group,and another 92 healthy examiners were enrolled as the control group.The serum NFL,S10Oβ,BBB index and mini-mental state examination(MMSE)score was compared between the two groups.The serum levels of NFL and Si0oβwere compared among patients with different BBB indexes and diferent MMSE scores.Pearson correlation analysiswas performed to analyze the correlation of serum NFL and S10Oβwith BBB index and MMSE score in patients with intracerebral hemorrhage.Results The serum levels of NFL,S100βand BBB index were increased and MMSE score were decreased in patients with intracerebral hemorrhage compared with control group(P<0.05).Compared with patients with BBB index<0.38,the serum levels of NFL and S100βwere significantly increased in patients with BBB index≥0.38(P<0.05).Compared with patients with MMSE score≥23.36,the serum levels of NFL and S100βwere significantly increased in patients with MMSE score<23.36(P<0.05).Correlation analysis indicating that serum levels of NFL and S10Oβwere positively correlated with BBB index,and negatively correlated with MMSE score(P<0.05).Conclusions The levels of NFL and S100B in peripheral blood were higher in patients with cerebral hemorrhage,and they were closely correlated with blood-brain barrier index and cognitive impairment,which is helpful to early detect the degree of brain injury and cognitive impairment in patients with cerebral hemorrhage and to predict the progress of the disease.
