复合小梁切除联合新鲜羊膜植入治疗青光眼的效果观察

摘　　要：目的分析复合小梁切除联合新鲜羊膜植入治疗青光眼的临床效果。方法抽取2020年1月至2022年6月南阳市眼科医院收治的青光眼患者58例,按随机数字表法分为对照组和观察组,每组29例。对照组予以复合小梁切除术治疗,观察组在对照组基础上联合新鲜羊膜植入治疗。比较两组临床效果、眼压、最佳矫正视力(BCVA)、血流动力学[阻力指数(RI)、眼动脉收缩峰值流速(PSV)、眼动脉舒张末期血流速度(EDV)]水平、角膜内皮功能[最小细胞面积(MIN)、内皮平均细胞面积(AVE)、角膜内皮细胞密度(CD)]水平。结果观察组治疗总有效率(96.55%,28/29)高于对照组(72.41%,21/29),P<0.05。术后6个月,观察组眼压水平低于对照组(P<0.05);术后1、6个月,观察组BCAV低于对照组(P<0.05);术后6个月,观察组RI水平低于对照组,PSV、EDV水平高于对照组(P<0.05);术后6个月,观察组MIN、AVE、CD水平高于对照组(P<0.05)。结论复合小梁切除联合新鲜羊膜植入治疗青光眼患者效果确切,可恢复血流动力学,降低眼压,改善角膜内皮功能和视力功能。Objective To analyze the clinical effect of compound trabeculectomy combined with fresh amniotic membrane implantation in the treatment of glaucoma.Methods Fifty-eight patients with glaucoma treated in Nanyang Eye Hospital from January 2020 to June 2022 were selected and divided into control group and observation group according to random number table method,with 29 cases in each group.The control group was treated by compound trabeculectomy,while the observation group was treated by fresh amniotic membrane implantation based on the treatment of the control group.The clinical efficacy,intraocular pressure,the best corrected visual acuity(BCVA),levels of hemodynamics,including resistance index(RI),peak systolic velocity of ophthalmic artery(PSV),and end-diastolic blood flow velocity of ophthalmic artery(EDV),the corneal endothelial function assessed by minimum cell area(MIN),mean endothelial cell area(AVE)and corneal endothelial cell density(CD)were compared between the two groups.Results The total effective rate(96.55%,28/29)of the observation group was higher than that of the control group(72.41%,21/29),P<0.05.Six months after surgery,the intraocular pressure level of the observation group was lower than that of the control group(P<0.05).One month and 6 months after surgery,the BCAV in the observation group was lower than that in the control group(P<0.05).Six months after surgery,the RI level of the observation group was lower than that of the control group,while the levels of PSV and EDV were higher than those of the control group(P<0.05).Six months after operation,the levels of MIN,AVE and CD in the observation group were higher than those in the control group(P<0.05).Conclusions Compound trabeculectomy combined with fresh amniotic membrane implantation has a definite effect in the treatment of glaucoma.It can restore the hemodynamics,decrease the intraocular pressure,and improve the corneal endothelial function and visual function.
