颞部皮脂腺癌1例

摘　　要：患者女,62岁,左颞部包块60年。皮肤科情况:左侧颞部可见一约3 cm×2 cm大小淡红色肿块,呈半球形,质硬,境界清楚,不可推动,表面粗糙,其上覆有黄白色颗粒,边缘有少许血痂,无糜烂、溃疡及渗出。皮损组织病理示:真皮内见大小不等和形态不规则的肿瘤团块,呈侵袭性生长,部分与表皮相连;瘤团由两种类型细胞构成:嗜碱性的基底样生发细胞及皮脂腺细胞,细胞异型性明显,核分裂象易见;部分瘤团中央见红染坏死物质。免疫组织化学示:肿瘤细胞突变型p53、CK、CK7均阳性;EMA皮脂腺细胞阳性;Ki67约20%阳性;GCDFP-15、CEA、S100、CD117均阴性。诊断:左颞部皮脂腺癌。治疗:标准外科手术切除全部皮损组织,病理结果示基底切缘未见癌累及。目前,该病例仍在随访中。A 62-year-old female presented with a left temporal mass for 60 years.Dermatological condition:there was a light red mass about 3 cm×2 cm in size in the left region,which was hemispherical,hard,clear and unpushable,with rough surface,covered with yellow and white particles and a little blood scab at the edge,without erosion,ulcer or exudation.Skin histopathology of dermis showed tumor masses of different sizes and irregular shapes,which were invasive and partly connected with the epidermis.The tumor mass consists of two types of cells showed basophilic basal-like germinal cells and sebaceous gland cells.Red stained necrotic material was seen in the center of some tumors.Immunohistochemistry showed that tumor cells mutated p53,CK and CK7 were positive.The sebaceous gland cells of EMA were positive.About 20% were positive for Ki67.Gcdfp-15,CEA,S100 and CD117 were negative.Diagnosis:left temporal sebaceous gland carcinoma.Treatment:All lesions were excised by standard surgery,and pathological results showed that no cancer was involved in the basal incised margin.The case is still being followed up.
