摘 要:目的:探讨骨皮质切开术对牙周炎致错位前牙行牙周-正畸患者牙根吸收的影响。方法:选取102例牙周炎致前牙错位患者为研究对象,根据治疗方法不同将患者分为对照组和观察组,每组各51例。对照组患者予以牙周-正畸治疗;观察组患者予以骨皮质切开术联合牙周-正畸治疗。比较两组患者牙周袋深度(PD)、附着丧失(AL)、牙龈沟出血指数(SBI)、覆颌度及覆盖度、尖牙移动速度及矫治时间、CBCT头颅测量指标[牙位置(U1I-CP、U1R-CP)、硬组织(UARA,Sd-CP)、软组织(NLA,Ls-HP)]及牙根吸收情况。结果:治疗后,观察组患者PD、AL、SBI均低于对照组(P<0.05);覆颌度、覆盖度较对照组改善(P<0.05);上颌、下颌尖牙移动速度快于对照组(P<0.05);矫治时间短于对照组(P<0.05);U1I-CP、U1R-CP、UARA、Sd-CP、NLA、Ls-HP均大于对照组(P<0.05);上颌牙列间隙关闭后尖牙牙根长度、牙槽骨总厚度大于对照组(P<0.05)。结论:骨皮质切开术联合牙周-正畸治疗可加快牙移动速度,改善深覆颌度及深覆盖度程度,缩短矫治时间,且可减少牙周-正畸治疗对患者牙根吸收的影响。Objective:To investigate the effect of corticotomy on root resorption in patients with periodontal-orthodontic anterior teeth caused by periodontitis.Methods:102 patients with anterior teeth dislocation caused by periodontitis were selected as the research objects.They were divided into control group and observation group according to different treatment methods,51 cases in each group.The control group was treated with periodontal orthodontic treatment,and the observation group was treated with corticotomy combined with periodontal orthodontic treatment.The periodontal pocket depth(PD),attachment loss(AL),gingival sulcus bleeding index(SBI),deep overjet,deep overjet,canine movement speed and correction time,CBCT head measurement indexes[Tooth position(U1I-CP,U1R-CP),hard tissue(UARA,Sd-CP),soft tissue(NLA,Ls-HP)]and root resorption were compared between the two groups.Results:After treatment,PD,AL and SBI in the observation group were lower than those in the control group(P<0.05).The deep overjet and deep coverage of the observation group were improved compared with the control group(P<0.05).The movement speed of maxillary and mandibular canines in the observation group was faster than that in the control group(P<0.05),and the treatment time in the observation group was shorter than that in the control group(P<0.05).The changes of U1I-CP,U1R-CP,UARA,Sd-CP,NLA and Ls-HP in the observation group were higher than those in the control group(P<0.05).After the maxillary dentition space was closed,the canine root length and total alveolar bone thickness in the observation group were higher than those in the control group(P<0.05).Conclusion:Corticotomy combined with periodontal-orthodontic treatment can significantly accelerate the speed of tooth movement,improve the degree of deep overjet and deep coverage,shorten the treatment time,and reduce the effect of periodontal-orthodontic treatment on root resorption.
