西酞普兰联合心境稳定剂治疗双相抑郁的临床疗效观察

摘　　要：目的 研讨西酞普兰联合心境稳定剂对双相抑郁的效果。方法 将2020年5月—2022年2月在厦门市仙岳医院接受治疗的120例双相抑郁症患者按用药的不同分为奥氮平组和西酞普兰组,2组均给予心境稳定剂(碳酸锂片)治疗,奥氮平组(60例)联合奥氮平治疗,西酞普兰组(60例)联合西酞普兰治疗,观察2组治疗效果、连线试验(TMT)评分、倍克-拉范森躁狂量表(BRMS)评分以及不良反应发生情况。结果 西酞普兰组治疗总有效率(96.67%)高于奥氮平组(83.33%)(P<0.05);经过治疗之后2组TMT-A、TMT-B时长均较之治疗前缩短,且西酞普兰组更短(P<0.05);治疗后西酞普兰组总转躁率(13.33%)较之奥氮平组(3.33%)更高P(<0.05),但西酞普兰组不良反应发生情况较之奥氮平组更少(P<0.05)。结论 西酞普兰联合心境稳定剂对于治疗双相抑郁症有着良好的治疗效果,不良反应发生情况较少,可以显著提高患者的认知功能,但患者会出现转躁的情况,在临床用药时需结合患者实际情况进行选择。Objective To discuss the effect of citalopram and mood stabilizer on bipolar depression.Methods A total of 120 patients with bipolar depression who received treatment in our hospital from May,2019 to February,2018 were divided into olanzapine group and citalopram group according to different administration.Among them,Olanzapine group(60 cases)received mood stabilizer(lithium carbonate tablet),and Citalopram group(60 cases)received citalopram combined with mood stabilizer.Mood stabilizer lithium carbonate tablets were used to observe the therapeutic effect,clinical indicators and the occurrence of adverse reactions in the two groups.Results After treatment,the overall response rate of citalopram(96.67%)was higher than olanzapine(83.33%)(P<0.05).After treatment,the citalopram group(13.33%)was higher than in the olanzapine group(3.33%)(P<0.05),but fewer adverse effects occurred in the citalopram group than in the olanzapine group(P<0.05).Conclusion Citalopram combined with mood stabilizer has a good therapeutic effect in the treatment of bipolar depression,with fewer adverse reactions,which can significantly improve the cognitive function of patients,but patients will experience mania,which should be selected according to the actual situation of patients in clinical use.
