仑伐替尼联合冷循环微波消融治疗肝细胞癌的效果及其对患者免疫功能的影响

摘　　要：目的探讨仑伐替尼联合冷循环微波消融治疗肝细胞癌的效果及其对患者免疫功能的影响。方法抽取2019年2月至2021年1月河南大学淮河医院收治的肝细胞癌患者96例,按照随机数字表法分为研究组与对照组,每组48例。对照组采用冷循环微波消融术治疗,研究组在对照组的基础上采用仑伐替尼治疗。比较两组疗效、血清甲胎蛋白(AFP)水平、肝功能[丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)、天门冬氨酸氨基转移酶(AST)]、免疫功能[T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))]水平及术后1年生存率。结果研究组总有效率(87.50%,42/48)高于对照组(68.75%,33/48),P<0.05;治疗后,研究组血清AFP水平低于对照组(P<0.05),研究组ALT、TBIL、AST水平低于对照组(P<0.05);研究组CD3^(+)、CD4^(+)水平高于对照组,CD8^(+)水平低于对照组(P<0.05)。研究组术后1年生存率(85.42%,41/48)高于对照组(64.58%,31/48),P<0.05。结论仑伐替尼联合冷循环微波消融治疗肝细胞癌疗效显著,能改善血清AFP水平和肝功能,提升机体免疫功能,延长患者生存期。Objective To investigate the efficacy of lenvatinib combined with cooled-tip microwave ablation on hepatocellular carcinoma patients and its effect on immune function.Methods A total of 96 patients with hepatocellular carcinoma treated in Huaihe Hospital of Henan University from February 2019 to January 2021 were selected,and they were divided into study group and control group according to random number table method,with 48 cases in each group.The control group was treated by cooled-tip microwave ablation,and the study group was treated with lenvatinib based on the treatment of the control group.The curative effect,level ofα-fetoprotein(AFP),liver function assessed by alanine transaminase(ALT),total bilirubin(TBIL)and aspartate transaminase(AST),immune function assessed by T lymphocyte subsets,including cluster of differentiation 3^(+)(CD3^(+)),cluster of differentiation 4^(+)(CD4^(+)),cluster of differentiation 8^(+)(CD8^(+)),and 1-year survival rate were compared between the two groups.Results The total effective rate of the study group(87.50%,42/48)was higher than that of the control group(68.75%,33/48),P<0.05.After treatment,the level of serum AFP in the study group was lower than that in the control group(P<0.05),the levels of ALT,TBIL and AST in the study group were lower than those in the control group(P<0.05).After treatment,the levels of CD3^(+)and CD4^(+)in the study group were higher than those in the control group,while the level of CD8^(+)in the study group was lower than that in the control group(P<0.05).The 1-year survival rate in the study group(85.42%,41/48)was higher than that in the control group(64.58%,31/48),P<0.05.Conclusions Lenvatinib combined with cooled-tip microwave ablation has significant therapeutic effects on hepatocellular carcinoma,which can improve level of serum AFP and liver function,enhance immune function,and prolong the survival period.
