顺铂联合替吉奥胶囊治疗晚期胃癌的近期临床效果

摘　　要：目的观察顺铂联合替吉奥胶囊治疗晚期胃癌的近期临床效果。方法选取2020年4月—2021年5月济南市中西医结合医院收治的晚期胃癌患者92例,采用“掷骰子”法分为观察组和对照组,各46例。对照组给予替吉奥胶囊治疗,观察组在对照组基础上联合注射用顺铂治疗,2组患者均连续治疗2个周期。比较2组近期疗效,治疗前后血清肿瘤指标[糖类抗原125(CA125)、神经元特异性烯醇化酶(NSE)]、血清肿瘤转移侵袭指标[乙酰肝素酶-1(HPA-1)、钙离子结合蛋白A4(S100A4)、腺苷酸活化蛋白激酶5(APK5)]及不良反应。结果观察组患者近期客观缓解率为67.39%,高于对照组的43.48%(χ^(2)=5.323,P=0.021)。治疗2个周期后,2组CA125、NSE、HPA-1、S100A4及APK5水平均较治疗前降低,且观察组低于对照组(P<0.01)。观察组不良反应总发生率为34.78%,低于对照组的58.70%(χ^(2)=5.283,P=0.021)。结论顺铂联合替吉奥胶囊治疗晚期胃癌的临床效果肯定,对于改善患者血清肿瘤指标和血清肿瘤转移侵袭指标具有积极影响,同时还能降低患者不良反应发生风险。Objective To observe the short-term effect of cisplatin combined with tegafur,gimeracil and oteracil potassium capsules in the treatment of advanced gastric cancer.Methods A total of 92 patients with advanced gastric cancer admitted to Jinan Hospital of Integrated Traditional Chinese and Western Medicine from April 2020 to May 2021 were selected,and they were divided into the observation group and the control group with 46 cases in each group.The control group was treated with tegafur,gimeracil and oteracil potassium capsules,and the observation group was treated with cisplatin for injection on the basis of the control group,both groups were treated for 2 consecutive cycles.The short-term efficacy of the two groups was compared,and the changes of serum tumor indicators(CA125,NSE),serum tumor metastasis and invasion indicators(HPA-1,S100A4,APK5)before and after treatment in the two groups were compared,as well as the occurrence of adverse reactions in the two groups were compared.Results The recent objective remission rate of the observation group was 67.39%,which was higher than 43.48%of the control group(χ^(2)=5.323,P=0.021).After 2 cycles of treatment,the levels of CA125,NSE,HPA-1,S100A4 and APK5 in the two groups were lower than those before treatment,and the observation group was lower than that in the control group(P<0.01).The total incidence of adverse reactions in the observation group was 34.78%,which was lower than 58.70%in the control group(χ^(2)=5.283,P=0.021).Conclusion The clinical effect of cisplatin combined with tegafur,gimeracil and oteracil potassium capsules in the treatment of advanced gastric cancer is worthy of affirmation.This scheme has a positive impact on improving the serum tumor indicators and serum tumor metastasis and invasion indicators of patients,and can also reduce the risk of adverse reactions of patients.
