摘　　要：T4期结直肠肿瘤侵犯范围广、易发生远处转移、治疗难度较大。新辅助治疗通过缩小肿瘤、降低分期,增加T4期结直肠癌手术切除机会。放疗可有效提高手术切除率,但会导致相关并发症,部分患者不能耐受。手术治疗作为T4期结直肠癌的首选治疗方法,可提高患者生存质量。靶向治疗属于新兴抗肿瘤方法。肠道微生态转化治疗通过调整肠道微生物群落的组成和功能影响肿瘤的发展达到治疗效果。多学科综合治疗作为一种新型治疗方法治疗效果更加显著。从新辅助治疗、放射治疗、手术治疗、靶向治疗、肠道微生物转化治疗、多学科综合治疗等方面综述T4期结直肠癌治疗方案,以期为临床多元化治疗提供参考,使患者获得最佳的个体化治疗效果和生存获益。T4 colorectal cancer tumor invasion is extensive,easy to occur distant metastasis,and difficult to treat.Neoadjuvant therapy increases the chance of surgical resection of T4 colorectal cancer by shrinking the tumor and reducing its staging.Radiotherapy can effectively improve the surgical resection rate,but it will lead to related complications that some patients can not tolerate.Surgical treatment is the preferred treatment for stage T4 colorectal cancer to improve the patients’quality of life.Targeted therapy is an emerging anti-tumor method.Intestinal microecological transformation therapy affects tumor development and therapeutic effect by adjusting the composition and function of intestinal microbial community.As a new type of treatment,multi-disciplinary integrative therapy makes the therapeutic effect more remarkable.In this paper,neoadjuvant therapy,radiation therapy,surgical resection,targeted therapy,intestinal microbial transformation therapy,and multidisciplinary comprehensive therapy were reviewed,in order to provide more valuable and diversified treatment programs for patients with stage T4 colorectal cancer,so that patients can obtain the best individualized treatment effect and survival benefits.
