达克罗宁胶浆对全麻拔牙术患者围术期应激反应及舒适度的影响

摘　　要：目的探讨盐酸达克罗宁胶浆对经鼻插管全麻拔牙患者围术期血流动力学、术后疼痛及麻醉复苏的影响。方法选取本院2022年6月至2022年12月择期全麻拔牙术患者60例作为研究对象,ASAⅠ或Ⅱ级,双盲随机分为含漱5 mL达克罗宁胶浆5 min并涂抹气管导管前端20 cm(实验组)和含漱5 mL生理盐水5 min并石蜡油涂抹气管导管前端20 cm(对照组),每组30例。两组予相同方式诱导经鼻插管,全麻维持及复苏,同一组医师完成手术。观察两组不同时段血流动力学及苏醒质量,术后VAS评分,舒适度评价。结果实验组在插管后1 min(T 1)、拔管后1 min(T 3)及拔管后30 min(T 4)时SBP、MAP显著低于对照组(P<0.05);实验组在术后1 h的VAS评分,术毕放气囊时呛咳及躁动程度显著低于对照组(P<0.05);实验组舒适度高于对照组,差异有统计学意义(P<0.05)。结论经鼻插管全麻拔牙术运用达克罗宁胶浆效果显著,有利于改善血流动力学,缓解术后短期疼痛,提高麻醉复苏质量。Objective To investigate the effects of Dyclonine mucilage on perioperative hemodynamics,postoperative pain,and anesthesia recovery in patients undergoing nasal intubation general anesthesia for tooth extraction.Methods Sixty ASAⅠorⅡpatients with dental extraction under general anesthesia from June to December 2022 in our hospital were enrolled and randomly divided into two groups:the experimental group,gargle with Dyclonine mucilage(5 ml)for 5 minutes and coatthe front end of the tracheal catheter(20 cm)with Dyclonine mucilage;and the control group,gargle with normal saline(5 ml)for 5 minutes and coat the front end of the tracheal catheter(20 cm)with paraffin oil,with 30 patients in each group.All patients were induced to undergo nasal intubation,general anesthesia maintenance,and resuscitation in the same way,and the same group of physicians completed the surgery.The hemodynamics and recovery quality,the postoperative VAS score,and comfort level were evaluated for the two groups at different time periods.Results The SBP and MAP in the experimental group were significantly lower than those in the control group at 1 min after intubation(T 1),1 min after extubation(T 3),and 30 min after extubation(T 4,P<0.05);The VAS score at 1 h after surgery in the experimental group was significantly lower than that in the control group(P<0.05);The comfort level of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Dyclonine mucilage can improve hemodynamics,relieve short-term postoperative pain,and improve the quality of anesthesia resuscitation in dental extraction under general anesthesia with nasal intubation.
