摘　　要：目的探讨自动乳腺全容积成像(ABVS)、超声弹性成像(UE)对乳腺肿瘤良恶性的鉴别诊断价值。方法选取114例乳腺肿瘤(共120个肿块)患者,均接受常规超声、ABVS、UE检查,以病理检查结果为金标准,评估常规超声、ABVS、UE以及ABVS+UE检查对乳腺肿瘤良恶性的鉴别诊断价值。结果病理检查结果显示,114例乳腺肿瘤患者中,良性患者45例,恶性患者69例;良性肿块48个,恶性肿块72个。常规超声、ABVS、UE、ABVS+UE检查诊断乳腺肿瘤良恶性的结果与病理检查结果的一致性Kappa值分别为0.27、0.60、0.53、0.90,其中ABVS+UE检查诊断乳腺肿瘤良恶性的结果与病理检查结果的一致性很高。ABVS+UE检查诊断乳腺恶性肿瘤的灵敏度、特异度、准确度均高于常规超声、ABVS、UE单独检查。结论ABVS+UE检查对乳腺肿瘤良恶性的鉴别诊断价值较高,优于单一检查,临床中可以根据实际情况联合应用多种检查,可有效提高临床诊断准确度。Objective To analyze the value of automated breast volume scanner(ABVS)and ultrasonic elastography(UE)in the differential diagnosis of benign and malignant breast tumors.Method A total of 114 patients with breast tumors(a total of 120 masses)were selected and underwent routine ultrasound,ABVS,and UE examination.The pathological examination results were used as the gold standard to evaluate the differential diagnostic value of routine ultrasound,ABVS,UE,and ABVS+UE examination for benign and malignant breast tumors.Result Pathological examination results showed that there were 114 patients with breast tumors,45 with benign tumors and 69 with malignant tumors;there were 48 benign masses and 72 malignant masses.The consistency between the results of conventional ultrasound,ABVS,UE,and ABVS+UE in diagnosing benign and malignant breast tumors and pathological examination was 0.27,0.60,0.53,and 0.90,respectively.Among them,the consistency between the results of ABVS+UE in diagnosing benign and malignant breast tumors and pathological examination was very high.The sensitivity,specificity,and accuracy of ABVS+UE examination in diagnosing breast malignant tumors were higher than those of conventional ultrasound,ABVS,and UE examination alone.Conclusion ABVS+UE examination has a high value in the differential diagnosis of benign and malignant breast tumors,which is superior to a single examination.In clinical practice,multiple examinations can be combined according to the actual situation,which can effectively improve the accuracy of clinical diagnosis.
