摘　　要：目的 探讨牙髓炎治疗前后龈沟液基质金属蛋白酶-8(MMP-8)/基质金属蛋白酶抑制剂-1(TIMP-1)、热休克蛋白-70(HSP-70)、高迁移率族蛋白1(HMGB1)水平变化,并分析其对治疗效果的预测价值。方法 选择2020年3—4月本院收治的行根管治疗的86例牙髓炎患者为研究对象,依据治疗4周后疗效分为有效组(74例)和无效组(12例)两组。比较两组临床症状指标[牙周袋深度(PD)、牙龈指数(GI)、菌斑指数(PLI)、出血指数(BI)、视觉模拟量表(VAS)]、龈沟液炎症因子[白细胞介素6(IL-6)、白细胞介素-1β(IL-1β)、白细胞介素-35(IL-35)、肿瘤坏死因子-α(TNF-α)]水平。分析两组龈沟液MMP-8/TIMP1、HSP-70、HMGB1水平。分析龈沟液各指标与临床症状指标、炎症因子水平相关性。分析龈沟液各指标(治疗2周后)对治疗效果的预测价值。结果 无效组PD、GI、PLI、BI、VAS、IL-6、IL-1β、IL-35、TNF-α水平均高于有效组(P<0.05);治疗2周后无效组MMP-8/TIMP1、HSP-70、HMGB1水平均高于有效组(P<0.05);MMP-8/TIMP1、HSP-70、HMGB1与GI、BI、VAS、IL-1β、TNF-α呈正相关(P<0.05);龈沟液各指标联合预测治疗效果的AUC大于单项预测(P<0.05)。结论 龈沟液MMP-8/TIMP1、HSP-70、HMGB1水平与牙髓炎治疗效果有关,且与龈沟炎症程度呈正相关,联合检测其水平对治疗效果具有一定预测价值。Objective To investigate the changes of matrix metalloproteinase-8(MMP-8)/matrix metalloproteinase-inhibitor-1(TIMP-1),heat shock protein-70(HSP-70)and high mobility group protein-1(HMGB1)levels in gingival creval fluid before and after treatment among patients with pulpitis,and analyze its value for predicting treatment effect.Methods A total of 86 patients suffered from pulpitis who received root canal treatment in Pingdingshan stomatological hospital from March 2020 to April 2022 were selected as the research objects.According to the therapeutic effect after 4 weeks,they were divided into effective group(74 cases)and ineffective group(12 cases).Clinical symptom indexes(periodontal pocket depth(PD),gingival index(GI),plaque index(PLI),bleeding index(BI),visual analogue scale(VAS)),inflammatory factors in gingival crevicular fluid[interleukin6(IL-6),interleukin-1β(IL-1β),interleukin-35(IL-35),and tumor necrosis factor-α(TNF-α)]were compared between the two groups.The levels of MMP-8/TIMP1,HSP-70 and HMGB1 in gingival crevicular fluid of the two groups were analyzed.The correlation betweenparameters in gingival crevicular fluid and clinical symptoms and inflammatory factors were analyzed.The predictive value of parameters in gingival crevicular fluid(2 weeks after treatment)for therapeutic effect was analyzed.Results The levels of PD,GI,PLI,BI,VAS,IL-6,IL-1β,IL-35 and TNF-αin ineffective group were higher than those in effective group(P<0.05).After 2 weeks of treatment,the levels of MMP-8/TIMP1,HSP-70 and HMGB1 in ineffective group were higher than those in effective group(P<0.05).MMP-8/TIMP1,HSP-70 and HMGB1 were positively correlated with GI,BI,VAS,IL-1βand TNF-α(P<0.05).The AUC of the combined predict value of gingival crevicular fluid parameters was greater than that of the single prediction(P<0.05).Conclusions The levels of MMP-8/TIMP1,HSP-70 and HMGB1 in gingival creval fluid are correlated with the therapeutic effect of pulpitis,and are positively correlated with the degree of gingival creval infla
