摘　　要：目的 通过对医学数字图像和通信(DICOM)协议的传输层进行安全加密,实现对医学影像数据从设备端输出开始的非侵入式、面向切面的网络链路层的安全管控,保证医学影像数据传输安全。方法 基于DICOM协议和零信任策略,结合加密和区块链技术,实施协议级数据加密传输。结果 构建了医学影像数据安全传输和管控系统,并在江苏省人民医院进行部署,实践了具备非侵入式、面向切面的传输链路层加固DICOM安全网关,建立了智能可信设备管理、身份认证管理、隐私脱敏、统一智能安全策略管理等安全体系。结论 医学影像数据传输安全和管控系统解决了DICOM协议的网络安全问题,保护了患者的个人敏感信息,在医疗卫生行业中具有一定的示范性和借鉴性。Objective By encrypting the transport layer of DICOM protocol,the security control of medical image data starting from the output of the device,non-intrusive and aspect-oriented network link layer is realized,and the security of medical image data transmission is realized.Methods Based on DICOM protocol and zero trust strategy,combined with encryption and blockchain technology,the protocol-level data encryption transmission was implemented.Results A medical image data secure transmission and control system is constructed and deployed in Jiangsu Province People's Hospital.The system has implemented the non-intrusive transmission link layer reinforcement DICOM security gateway,and established the intelligent trusted device management,identity authentication management,privacy desensitization,unified intelligent security policy management and other security systems.Conclusion The security and control system of medical image data transmission solves the network security problem of DICOM protocol,protects the patient's personal sensitive information,and has certain demonstration and adoption in the medical and health industry.
