两种止血材料对口服阿司匹林的老年拔牙患者止血作用的对比研究

摘　　要：目的对比胶原塞和明胶海绵在口服阿司匹林的老年拔牙患者术后辅助止血的效果。方法将124例口服阿司匹林的老年拔牙患者随机平均分为试验组和对照组。试验组患者在拔牙后使用胶原塞进行拔牙窝填塞,随后缝合牙龈止血+棉卷压迫拔牙窝止血。对照组使用明胶海绵填塞拔牙窝,其余止血步骤同试验组。分别在术后15、30、45和60 min时观察两组患者拔牙窝凝血情况。在拔牙48 h后,患者对此次拔牙就诊过程进行就诊满意度评价。结果试验组患者在45 min内均得到了有效止血,对照组患者在60 min内均得到了有效止血。试验组患者在15和30 min时的达到止血的数目多于对照组(P<0.05),在45和60 min时的达到止血的数目与对照组相比无统计学差异(P>0.05)。试验组在拔牙就诊满意度方面优于对照组(P<0.05)。结论对口服阿司匹林的老年拔牙患者术后使用胶原塞辅助止血效果优于明胶海绵,同时可提高患者就诊满意度。Objective To compare the effect of collagen plug and gelatin sponge in assisting hemostasis on postoperative hemostasis in elderly patients with oral aspirin for tooth extraction.Methods 124 elderly patients with oral aspirin for tooth extraction were randomly divided into experimental group and control group.In experimental group,the collagen plug was used to fill the tooth socket after tooth extraction,then suture gingival hemostasis+cotton roll compression of tooth extraction fossae hemostasis.In the control group,the gelatin sponge was used to fill the tooth sockets.At 15/30/45 and 60 minutes after operation,the blood coagulation of tooth extraction in two groups was observed.48 hours after the tooth extraction,the patients evaluated the satisfaction of the tooth extraction.Results All patients in the experimental group were effectively hemostatic within 45 minutes,while those in the control group were effectively hemostatic within 60 minutes.The number of patients with hemostasis at 15 min and 30 min in the experimental group was higher than that in the control group(P<0.05),the number of hemostasis achieved at 45 minutes and 60 minutes was not statistically different from the control group(P<0.05).The satisfaction of treatment for dental extraction in experimental group was higher than that in control group((P<0.05).Conclusion The effect of the collagen stopper is better than the gelatin sponge in elderly tooth extraction patients who take aspirin orally after surgery,and can improve the satisfaction of patients.
