摘 要:目的观察评估钛锆合金3.3 mm/4.1 mm直径种植体分别用于牙周炎患者水平骨量轻、中度不足的前磨牙/磨牙区并采用单冠固定修复2年后的临床效果和种植体周边缘骨水平变化情况,以期为临床提供参考。方法选自2018年5月至2021年7月在北京大学口腔医院牙周科行后牙种植治疗并伴种植区水平骨量不足的牙周炎患者30名。在前磨牙/磨牙分别植入直径3.3 mm/4.1 mm的Roxolid?钛锆合金种植体共47颗。术后4个月采用螺丝固位氧化锆全瓷冠修复并负重。种植术后3个月、修复后即刻、3个月、6个月、1年及2年进行临床检查和维护治疗,种植术后即刻、修复后即刻、1年及2年进行影像学检查。通过问卷记录患者术后反应。使用广义线性混合模型比较不同随访时间点各临床指标及边缘骨水平的变化。结果共25名患者、40颗种植体完成2年复查。所有病例术后肿痛反应轻微。与修复体戴入即刻的基线比较,负重1年及2年后边缘骨水平变化量极少,分别为-0.02±0.14 mm、0.01±0.17mm;种植体周黏膜炎发生率分别为23.8%、18.4%;无种植体周炎。机械学并发症发生率2.5%。种植体成功率100%。结论对于后牙区水平骨量轻、中度不足的牙周炎患者,钛锆合金3.3 mm/4.1 mm直径种植体分别用于前磨牙/磨牙是简便可行的治疗选择。2年随访期内边缘骨水平保持稳定,机械学并发症发生率较低,软组织相关指标基本稳定。Objective To evaluate clinical outcomes and peri-implant marginal bone level of Ti-Zr alloy implants with diameters of 3.3 mm/4.1 mm,which were placed in periodontitis patients’premolar/molar regions presenting mild-to-moderate horizontal bone volume deficiency,at 2 years after loading with single crowns.Methods 30 periodontitis patients who had posterior teeth loss with insufficient horizontal bone volume were recruited from May 2018 to July 2021 at Department of Periodontology,Peking University School and Hospital of Stomatology.Roxolid®Ti-Zr alloy implants with diameters of 3.3 mm/4.1 mm were placed in premolar/molar regions,respectively.A total of 47 implants were placed and loaded with screw-retained zirconia all-ceramic single crowns 4 months later.Clinical examinations and supportive periodontal therapy were implemented at 3 months after implant placement,loading,and 3 months,6 months,1 year and 2 years after loading.Radiographic examinations were implemented at implant placement,loading,1 year and 2 years after loading.Post-operative reactions of the patients were recorded by Numerical rating scale form.Clinical parameters and changes of marginal bone level were compared at different follow-up time points with a general linear mixed model.Results A total of 25 patients and 40 implants completed the 2-year evaluation.Mild post-operative reactions were reported in all patients.At the time points of 1 year and 2 years after loading,changes of marginal bone level were−0.02±0.14mm and 0.01±0.17mm respectively.The incidence rate of peri-implant mucositis was 23.8%and 18.4%respectively.No peri-implantitis was observed.The prevalence of technical complications of implants was 2.5%.The success rate was 100%.Conclusions For periodontitis patients with mild-to-moderate insufficient horizontal bone volume in posterior regions,placing Ti-Zr alloy implants with diameters of 3.3 mm/4.1 mm supporting single crowns in premolar/molar regions is a feasible treatment option.Over a 2-year follow-up period,the marginal
