摘 要:目的 探索血小板相关参数与视网膜静脉阻塞(retinal vein occlusion,RVO)发生的相关性,进而明确其作为预测RVO发生的可行性。方法 回顾性选取2018年1月至2020年12月三峡大学附属仁和医院收治的82例RVO患者作为RVO组,同期选取82例健康体检者作为正常对照组。比较两组受试者的基础疾病情况、视力、眼压、红细胞计数(red blood cell,RBC)、血红蛋白(hemoglobin,HGB)外周血血小板计数(platelet,PLT)、平均血小板体积(mean platelet volume,MPV)、血小板分布宽度(platelet distribution width,PDW)以及血小板压积(plateletcrit,PCT)等实验室检查指标。结果 RVO组高血压及高脂血症的患病率均显著高于对照组(P<0.05);两组间眼压差异无统计学意义,对照组视力优于RVO组(P<0.01);RVO组患者RBC及HGB均显著低于对照组(P<0.01)。与对照组比较,RVO组血小板相关参数差异均有统计学意义(P<0.01)。Logistic回归分析结果显示,高血压、MPV及PDW为RVO发生的独立危险因素(P<0.05);MPV预测RVO发生的曲线下面积为0.696(95%CI:0.616~0.776,P<0.01),敏感度为52.44%,特异性为81.71%,最佳诊断截值为9.95fl,而PDW预测RVO发生的曲线下面积为0.856(95%CI:0.799~0.914,P<0.01),敏感度为73.17%,特异性为87.80%,最佳诊断截值为16.10%。结论 血小板相关参数MPV与PDW为RVO发生的独立危险因素,在预测RVO发生方面PDW优于MPV。Objective To investigate the relationship between platelet-related parameters and retinal vein occlusion(RVO),and clarify its feasibility for predicting the occurrence of RVO.Methods A total of 82 patients with RVO admitted to Renhe Hospital of Three Gorges University from January 2018 to December 2020 were retrospectively selected as the RVO group.In the same period,82 healthy physical examination people without age and gender differences were selected as the normal control group.The basic diseases,visual acuity,intraocular pressure,red blood cell(RBC),hemoglobin(HGB),peripheral blood platelet(PLT)count,mean platelet volume(MPV),platelet distribution width(PDW) and plateletcrit(PCT) and other laboratory test indicators were compared.Results The prevalence of hypertension and hyperlipidemia in RVO group were significantly higher than those in control group(P<0.05);there was no difference in intraocular pressure between the two groups,and the visual acuity in control group was better than RVO group(P<0.01);RBC and HGB in RVO group were significantly lower than those in control group(P<0.01),while platelet related parameters showed significant difference compared with control group(P<0.01).Logistic regression analysis showed that hypertension,MPV and PDW were independent risk factors for RVO(P<0.05);the area under the curve of RVO predicted by MPV was 0.696(95%CI:0.616-0.776,P<0.01),the sensitivity was 52.44%,the specificity was 81.71%,the best cut-off point was 9.95fl,and the area under the curve of RVO predicted by PDW was 0.856(95%CI:0.799-0.914,P<0.01),the sensitivity was 73.17%,the specificity was 87.80%,and the best cut-off point was 16.10%.Conclusion MPV and PDW of platelet-related parameters are independent risk factors for RVO,and PDW is better than MPV in predicting the occurence of RVO.
