摘 要:目的探讨显微支撑喉镜低温等离子射频消融治疗早期声门型喉癌的效果。方法选取74例早期声门型喉癌患者,随机分为显微镜组和切除术组,各37例。切除术组采用单纯喉部分切除术治疗,显微镜组采用显微支撑喉镜低温等离子射频消融治疗。对比2组围术期情况、黏膜恢复评分、疼痛视觉模拟(VAS)评分、嗓音声学功能、炎性因子及并发症。结果和切除术组对比,显微镜组术中出血量较少,手术时间、住院时间均较短;术后4周黏膜恢复评分较高,术后1 d VAS评分较低;术后6个月显微镜组基频微扰、标准化噪声能力、振幅微扰均较低;术后1 d显微镜组C反应蛋白(CRP)、白介素-6(IL-6)水平均较低,白介素-2(IL-2)水平较高(P<0.05)。显微镜组并发症总发生率为2.70%(1/37),较切除术组24.32%(9/37)低(P<0.05)。结论对早期声门型喉癌患者采用显微支撑喉镜低温等离子射频消融治疗,可减少术中出血量,减轻炎性应激,加快患者术后恢复,并可改善嗓音声学功能,且安全性更高。Objective To analyze the effect of low temperature plasma radiofrequency ablation with micro support laryngoscope in the treatment of early glottic laryngeal cancer.Methods 74 patients with early glottic laryngeal cancer were randomly divided into microscope group and resection group,with 37 cases in each group.The resection group was treated with simple partial laryngectomy,and the microscope group was treated with low-temperature plasma radiofrequency ablation with Microsupport laryngoscope.The perioperative conditions,mucosal recovery score,visual analogue of pain(VAS)score,voice acoustic function,inflammatory factors and complications were compared between the 2 groups.Results Compared with the resection group,the operation time and hospital stay in the microscope group were shorter,and the amount of intraoperative bleeding was less;The mucosal recovery score was higher at 4 weeks after operation,the VAS score was lower at 1 day after operation,and the fundamental frequency perturbation,standardized noise ability and amplitude perturbation were lower in the microscope group at 6 months after operation;1 day after operation,the levels of C-reactive protein(CRP)and interleukin-6(IL-6)in the microscope group were lower,and the levels of interleukin-2(IL-2)were higher(P<0.05);The incidence of complications in the microscope group was 2.70%(1/37)lower than that in the resection group 24.32%(9/37)(P<0.05).Conclusion Low temperature plasma radiofrequency ablation with Microsupport laryngoscope for patients with early glottic laryngeal cancer can reduce intraoperative bleeding,reduce inflammatory stress,accelerate postoperative recovery,improve voice acoustic function and have higher safety.
