不同化疗方案治疗多发性骨髓瘤的疗效及并发感染的对比研究

摘　　要：目的探讨不同治疗方案(VP方案和VAD方案)治疗多发性骨髓瘤患者的疗效及感染发生情况,以期为个性化诊疗方案的制定提供参考依据。方法于2019年5月至2021年5月采用定群抽样的方法选取研究对象,结合横断面观察和随访研究对所有进入研究队列的患者进行随访及调查,并对调查结果进行进一步分析和对比。结果共纳入符合要求的病例60例,遵循随机化原则分配给VP治疗组以及VAD治疗组,各30例。VP治疗组的骨痛、骨骼变形、贫血和出血、神经系统症状、细菌感染以及肾功能损害临床改善效果更佳(P<0.05)。感染发生曲线结果显示VP治疗组与VAD治疗组之间的感染发生时间及感染发生率的差异均具有统计学意义(P<0.05)。结论VP方案在治疗多发性骨髓瘤中总体效果优于VAD方案,但仍需要进行个性化制定。Objective To explore the comprehensive value of different treatment options(VP and VAD)for the treatment of multiple myeloma patients,so as to provide reference for the development of personalized treatment plans.Methods A cluster-sampling method was used to select study object,and cross-sectional observation and follow-up studies were used to investigate the characteristics of patients entering the study cohort.Follow-up and investigation were conducted,and the results of the investigation were further analyzed and compared.Results A total of 60 eligible cases were included,and 30 cases were allocated to VP treatment group and VAD treatment group according to the principle of randomization.The VP treatment group had better clinical improvement effect on bone pain,bone deformation,anemia and bleeding,nervous system symptoms,bacterial infection and renal function damage(P<0.05).The results of infection occurrence curve showed that the difference of infection occurrence time and infection incidence between VP treatment group and VAD treatment group was statistically significant(P<0.05).Conclusion VP regimen is superior to VAD regimen in the treatment of multiple myeloma,but it still needs to be personalized.
