腹腔镜辅助小切口胃癌根治术对进展期胃癌患者手术相关指标及术后胃肠功能恢复的影响

摘　　要：目的研究腹腔镜辅助小切口胃癌根治术对进展期胃癌患者手术相关指标及术后胃肠功能恢复的影响。方法选择进展期胃癌患者96例作为观察对象,按随机数字表法分成2组,各48例。对照组行传统的开腹胃癌根治术治疗,研究组行腹腔镜辅助小切口胃癌根治术。对比两组临床疗效、手术相关指标、术后胃肠道功能恢复情况及并发症发生率。结果研究组治疗总缓解率高于对照组,差异有统计学意义(P<0.05);两组手术操作时间、术中淋巴结清扫数量对比,差异无统计学意义(P>0.05);研究组术中出血量较对照组更少,术后住院时间较对照组更短,差异有统计学意义(P<0.05);研究组术后胃肠功能恢复效果较对照组更优,差异有统计学意义(P<0.05);研究组术后并发症总发生率较对照组更低,差异有统计学意义(P<0.05)。结论腹腔镜辅助小切口胃癌根治术治疗进展期胃癌的效果确切,能有效清除肿瘤病灶,且存在术中出血量低、术后并发症发少等优势,有利于术后胃肠功能尽早恢复,可于临床推广。Objective To study the effect of laparoscopic assisted small incision radical gastrectomy on surgical related indexes and postoperative gastrointestinal function recovery in patients with advanced gastric cancer.Methods 96 patients with advanced gastric cancer were selected and divided into 2 groups using a random number table method,with 48 patients in each group.The control group received traditional open radical gastrectomy for gastric cancer,and the study group received laparoscopic assisted small incision radical gastrectomy for gastric cancer.The clinical efficacy,operation related indexes,postoperative gastrointestinal function recovery and complication rate were compared between the 2 groups.Results The total remission rate in the study group was significantly higher than that in the control group(P<0.05);There was no significant difference in the operation time and the number of lymph node dissections between the 2 groups(P>0.05);The study group had less intraoperative bleeding and shorter postoperative hospital stay than the control group,with statistically significant difference(P<0.05);The recovery effect of gastrointestinal function in the study group was better than that in the control group,with statistically significant difference(P<0.05);The total incidence of postoperative complications in the study group was less than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic assisted small incision radical gastrectomy for advanced gastric cancer is effective in the treatment of advanced gastric cancer.It can effectively clear the tumor focus,and has the advantages of low intraoperative bleeding and less postoperative complications.It is conducive to the early recovery of gastrointestinal function after operation and can be popularized in clinical practice.
