微波深部热疗联合区域灌注化疗治疗晚期胰腺癌的临床研究

摘　　要：目的研究微波深部热疗联合区域灌注化疗治疗晚期胰腺癌的临床疗效。方法选取2016年1月至2017年1月本院治疗的90例晚期胰腺癌患者作为研究对象,随机分为观察组与对照组,每组45例。对组组采用区域灌注化疗,观察组采用微波深部热疗联合区域灌注化疗,比较两组临床疗效、临床症状缓解率、卡诺夫斯凯计分(KPS)评分提升>15分占比、生存率及不良反应发生率。结果观察组治疗总有效率为75.56%,高于对照组的37.78%,差异有统计学意义(P<0.05)。观察组黄疸、腹水、腹痛腹胀缓解率及KPS评分提升>15分占比均高于对照组,差异有统计学意义(P<0.05)。观察组治疗后6、12、18个月的生存率均高于对照组,差异有统计学意义(P<0.05);两组治疗后2、3、5年生存率比较差异无统计学意义。两组不良反应发生率比较差异无统计学意义。结论微波深部热疗联合区域灌注化疗治疗晚期胰腺癌临床疗效显著,可有效改善患者临床症状,延长生存期,且无严重不良反应,值得临床推广应用。Objective To explore the clinical study of microwave deep hyperthermia combined with regional perfusion chemotherapy for advanced pancreatic cancer.Methods 90 patients with advanced pancreatic cancer treated in our hospital from January 2016 to January 2017 were selected as research subjects,and they were randomly divided into the observation group and the control group,with 45 cases in each group.The control group was treated with regional perfusion chemotherapy,and the observation group was treated with microwave deep hyperthermia combined with regional perfusion chemotherapy,the clinical efficacy,clinical symptom remission rate,Kanofsky performance score(KPS)improvement rate of>15 scores,survival rate and incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group was 75.56%,which was higher than 37.78%in the control group,and the difference was statistically significant(P<0.05).The percentages of relief rates of jaundice,ascites,abdominal pain and distention and KPS score>15 scores in the observation group were higher than the control group,and the differences were statistically significant(P<0.05).The survival rates at 6,12 and 18 months after treatment in the observation group were higher than the control group,the differences were statistically significant(P<0.05).There were no significant differences in the survival rates of 2,3 and 5 years after treatment between the two groups.There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Microwave deep hyperthermia combined with regional perfusion chemotherapy has significant clinical efficacy in the treatment of advanced pancreatic cancer,which can effectively improve clinical symptoms and prolong survival without serious adverse reactions,and is worthy of clinical promotion and application.
