三参通络汤联合西药治疗对中风病急性期气虚血瘀证患者中医证候及神经功能的影响

摘　　要：目的探讨三参通络汤联合西药治疗在中风病急性期气虚血瘀证中的应用效果。方法选取2020年1月—2023年1月济宁市中医院收治的86例中风病急性期患者,按随机数表法分为两组,各43例。对照组予以西药治疗,观察组加用三参通络汤治疗,持续14 d。比较两组临床疗效、中医证候积分、神经功能、脑血流变化及不良反应。结果观察组总有效率为95.35%,较对照组的81.40%高,差异有统计学意义(χ^(2)=4.074,P<0.05)。治疗后,观察组半身不遂、口舌歪斜、言语謇涩、气短乏力积分为低于对照组,差异有统计学意义(P<0.05);观察组治疗后美国国立卫生研究院卒中量表(National Institute of Health Stroke Scale,NIHSS)评分低于对照组,大脑中动脉平均血流速度(mean velocity,V_(m))、舒张末期流速(diastolic velocity,V_(d))、流速峰值(systolicvelocity,V_(s))高于对照组,差异有统计学意义(P<0.05)。两组不良反应相比,差异无统计学意义(P>0.05)。结论三参通络汤联合西药能增强中风病急性期疗效,减轻神经功能损伤,增加脑血流量,降低中医证候积分,安全可靠。Objective To explore the application effect of Sanshen Tongluo decoction combined with western medicine in the acute phase of stroke with Qi deficiency and blood stasis.Methods Eighty-six patients with acute stage of stroke disease admitted to Jining Hospital of Traditional Chinese Medicine from January 2020 to January 2023 were selected and divided into two groups of 43 cases each according to the randomized numerical table method.The control group was treated with western medicine,and the observation group was treated with additional Sanshen Tongluo decoction for 14 d.Compared the two groups'clinical efficacy,Chinese medicine symptoms points,neurological function,cerebral blood flow changes and adverse reactions.Results The total effective rate of the observation group was 95.35%,which was higher than that of the control group(81.40%),and the difference was statistically significant(χ^(2)=4.074,P<0.05).The points of hemiplegia,crooked mouth and tongue,speech and shortness of breath after treatment in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The National Institutes of Health Stroke Scale(NIHSS)score of the observation group was lower than that of the control group,and the mean velocity(V_(m)),diastolic velocity(V_(d))and systolic velocity(V_(s))of the middle cerebral artery were higher than those of the control group,and the difference was statistically significant(P<0.05).Adverse reactions in the two groups were compared,and the difference was not statistically significant(P>0.05).Conclusion Sanshen Tongluo decoction combined with western medicines can enhance the efficacy of acute phase of stroke disease,reduce neurological function damage,increase cerebral blood flow,and reduce the TCM evidence points,which is safe and reliable.
