摘　　要：目的探讨肉芽肿性乳腺炎(granulomatous mastitis,GM)和浸润性乳腺癌的MRI特征。方法选取经病理证实的GM患者28例(GM组)和28例浸润性乳腺癌(浸润性乳腺癌组)进行对照,以患者健侧乳腺对应区域作为正常乳腺腺体组进行阴性对照正常乳腺腺体。采用GE Discovery 7503.0T MR扫描仪,专用双穴乳腺相控阵表面线圈进行扫描。扫描序列包括:常规T_(1)WI、T_(2)WI、DWI、DCE等。比较GM和浸润性乳腺癌ADC值、增强曲线等各种影像学特征的差异。结果浸润性乳腺癌ADC值低于GM,GM的ADC值低于正常乳腺腺体,差异有统计学意义(P<0.05);而GM、浸润性乳腺癌增强曲线类型、各期强化程度、DWI信号特征比较差异无统计学意义(P>0.05)。结论ADC值的客观测量对鉴别GM和浸润性乳腺癌具有重要价值,而增强曲线类型对二者的鉴别价值受限。Objective To study the MR imaging features of granulomatous mastitis and invasive breast cancer.Methods Twenty eight patients with granulomatous mastitis and 28 patients with invasive breast cancer confirmed by pathology were collected for control study.The corresponding area of the healthy breast was taken as the negative control group.All patients were scanned with the special breast phased array coil of Ge discovery 7503.0T magnetic resonance instrument.Scanning sequences included conventional T_(1)WI,T_(2)WI,DWI,DCE,etc.The differences of ADC values and enhancement curves were compared between granulomatous mastitis and invasive breast cancer.Results The ADC value of granulomatous mastitis was higher than that of invasive breast cancer and lower than that of normal breast gland.The difference among the three groups was statistically significant(P<0.05);However,there was no significant difference between granulomatous mastitis and invasive breast cancer in the type of enhancement curve,degree of enhancement at each stage and DWI signal characteristics(P>0.05).Conclusion Objective measurement of ADC value is of great value in differentiating granulomatous mastitis and invasive breast cancer,while,enhancement curve type has limited value in differentiating the two diseases.
