摘　　要：目的研究Nd:YAG激光术治疗白内障术后囊袋收缩综合征(capsular constriction syndrome,CCS)患者对视力和屈光度的改善、临床效果及预后的影响。方法选择CCS患者200例(220眼)为研究对象。按照随机数字表法分为试验组109例(112眼)和对照组91例(108眼)。对照组行手术剥离式前囊口松解,试验组给予Nd:YAG激光术治疗。比较2组治疗前、治疗后1周的视力(best corrected visual acuity,BCVA)、屈光度、眼压(intraocular pressure,IOP)和黄斑体积(macular volume,MV)、囊口直径与眼内散光量。2组均于术后1个月进行随访,分析并发症发生率情况。结果治疗后1周,2组视力、屈光度、IOP、MV、囊口直径、眼内散光量优于治疗前,试验组视力、屈光度、IOP、MV、囊口直径、眼内散光量优于对照组(P<0.05);试验组总体并发症发生率低于对照组(P<0.05)。结论Nd:YAG激光术治疗CCS患者能够有效提升视力程度、改善屈光度情况,维持眼压及黄斑体积,缓解囊袋收缩,对预后有积极影响,且具有一定的临床安全性。Objective To investigate the effect of Nd:YAG laser therapy on visual acuity and diopter improvement,clinical effect and prognosis of patients with capsular contraction syndrome(CCS)after cataract surgery.Methods A total of 200 patients(220 eyes)with CCS were selected as the research objects,and divided into the experimental group(109 patients,112 eyes)and the control group(91 patients,108 eyes)according to random number table method.The control group underwent surgical dissection of the anterior capsule opening,and the experimental group received Nd:YAG laser treatment.Best corrected visual acuity(BCVA),diopter,intraocular pressure(IOP)and macular volume(MV),diameter of capsule opening and ocular residual astigmatism were compared between two groups before treatment and at 1 week after treatment.Patients in both groups were followed up at 1 month after surgery to analyze the incidence of complications.Results At 1 week after treatment,the visual acuity,diopter,IOP,MV,diameter of capsule opening and ocular residual astigmatism of the two groups were better than those before treatment,and the visual acuity,diopter,IOP,MV,diameter of capsule opening and ocular residual astigmatism of the experimental group were better than those of the control group(P<0.05).The overall complication rate of experimental group was lower than that of control group(P<0.05).Conclusion Nd:YAG laser therapy for CCS patients can effectively improve visual acuity and diopter,maintain IOP and macular volume,and alleviate capsular contraction,which has a positive impact on prognosis and has certain safety in clinical practice.
