摘 要:目的研究止痛健骨方对兔膝骨关节炎(KOA)关节软骨中Wnt5a表达的影响及关节软骨中Wnt5a含量与关节软骨T2值的相关性,探讨T2 mapping成像评估治疗疗效的可行性。方法将32只白兔随机分为正常组、模型组、硫酸氨基葡萄糖组和止痛健骨方组,每组8只。除正常组外,其余组白兔均采用改良伸直制动法建立KOA模型。制动2周后,止痛健骨方组给予止痛健骨方2.98 g(生药量)/(kg·d)灌胃,硫酸氨基葡萄糖组给予硫酸氨基葡萄糖0.088 g/(kg·d)灌胃,正常组和模型组给予蒸馏水灌胃,均1次/d,连续灌胃4周。实验结束后,对兔行右膝关节T2 mapping成像,记录各组关节软骨T2值;取兔右膝关节软骨,番红固绿染色观察软骨损伤情况,免疫组化法检测软骨中Wnt5a表达情况,Spearman相关分析关节软骨中Wnt5a表达量与关节软骨T2值的相关性。结果模型组、止痛健骨方组、硫酸氨基葡萄糖组关节软骨T2值和关节软骨中Wnt5a表达量均明显高于正常组(P均<0.05),止痛健骨方组、硫酸氨基葡萄糖组关节软骨T2值和关节软骨中Wnt5a表达量均明显低于模型组(P均<0.05),止痛健骨方组关节软骨中Wnt5a表达量明显高于硫酸氨基葡萄糖组(P<0.05)。模型组、硫酸氨基葡萄糖组、止痛健骨方组细胞间基质及软骨层有不同程度的破坏或丢失,硫酸氨基葡萄糖组、止痛健骨方组上述病变较模型组轻。相关性分析显示关节软骨中Wnt5a表达量与T2值呈强正相关(r=0.884,P<0.05)。结论止痛健骨方可能通过下调关节软骨中Wnt5a表达而发挥延缓KOA软骨损伤的作用;软骨中Wnt5a表达量与T2 mapping成像有较好的相关性,T2 mapping成像有助于无创评估止痛健骨方对关节软骨损伤的影响。Objective It is to study the effects of decoction for relieving pain and fortifying bone(DRPFB)on the expression of Wnt5a in articular cartilage of rabbits with knee osteoarthritis(KOA),to preliminarily clarify the correlation between the content of Wnt5a and T2 value in articular cartilage,and to explore the feasibility of T2 mapping imaging in evaluation of the efficacy of this decoction on KOA.Methods Thirty-two white rabbits were randomly divided into normal group,model group,glucosamine sulfate group and DRPFB group,with 8 rabbits in each group.Except for the normal group,the KOA models were established by modified extension braking method in the rabbits of the other groups.After 2 weeks of braking,the DRPFB group was given DRPFB 2.98 g(raw drug amount)/(kg·d)by gavage,the glucosamine sulfate group was given glucosamine sulfate 0.088 g/(kg·d)by gavage,the normal group and model group were given while distilled water by gavage,all once daily,continuously treated for 4 weeks.At the end of the experiment,T2 mapping imaging of the right knee joint was performed on the rabbits,and the T2 values of articular cartilage in each group were recorded;the cartilage of the right knee joint of the rabbits was taken to stain with Safranin-Fast green staining to observe the damage of cartilage,the expression of Wnt5a in cartilage was detected by immunohistochemistry,and the correlation between the expression of Wnt5a in articular cartilage and the T2 values of articular cartilage was analyzed by Spearman’s correlation.Results The T2 value of articular cartilage and the expression of Wnt5a in articular cartilage were significantly higher in the model group,DRPFB group and glucosamine sulfate group than those in the normal group(all P<0.05),the T2 value of articular cartilage and the expression of Wnt5a in articular cartilage in the DRPFB group and glucosamine sulfate group were significantly lower than those in the model group(all P<0.05),and the expression of Wnt5a in articular cartilage in the DRPFB group was significa
