佩戴过渡性弹性义齿对前牙缺失患者心理状况的影响

摘　　要：目的:评价前牙缺失患者佩戴过渡性弹性义齿对其社交回避及苦恼、焦虑以及口腔健康相关生活质量的影响。方法:选取拔除前牙后择期修复的患者114例,随机分为义齿组(n=52)和对照组(n=52),两组患者于拔牙后2周填写社交回避及苦恼量表(SAD),焦虑自评表(SAS)及口腔健康影响程度量表(OHIP-14)。拔牙后2周义齿组开始佩戴弹性义齿,对照组不佩戴弹性义齿仅常规复查。两组于拔牙后4周再次填写上述量表。结果:拔牙后2周义齿组SAD、SAS以及OHIP-14得分与对照组间差异无统计学意义(均P>0.05)。拔牙后4周,义齿组3种量表得分均低于对照组(P<0.05)。义齿组拔牙后4周较拔牙后2周3种量表得分的差值高于对照组(P<0.05)。结论:前牙缺失患者在修复治疗过程中佩戴过渡性弹性义齿可有效提高患者口腔相关生活质量,改善日常社交问题,减少焦虑情绪。Objective:To evaluate the effect of transitional elastic dentures wearing on social avoidance and distress,anxiety and oral health-related quality of life in patients with anterior teeth loss.Methods:A total of 114 patients with extracted anterior teeth and elective prosthetic treatment were divided into denture group(n=52)and control group(n=52)randomly.Two groups of patients were instructed to complete the Social Avoidance and Dis-tress Scale(SAD),Self-Rating Anxiety Scale(SAS)and Oral Health Impact Scale(OHIP-14)at 2 weeks after ex-traction.The denture group began to wear elastic dentures at 2 weeks after tooth ex traction,and the control group only received routine review.The two groups filleld in the above 3 scales again at 4 weeks after tooth extraction.Results:There was no significant difference in the scores of SAD,SAS and OHTP-14 between the denture group and the control group at 2 weeks after extraction.The scores of SAD,SAS and OHIP-14 were lower in the denture group than in the control group at 4 weks after tooth extraction(P<0.05).The difference value of the scores of the 3 scales filled at 2 weeks and 4 weeks after extraction was higher in the denture group than in the con-trol group(P<0.05).Conclusion:It suggests that wearing transitional elastic dentures during prosthetic treatment for patients with anterior teeth loss could effectively improve their oral related quality of life,improve their daily social interaction,and reduce anxiety.
