白内障术前预防性使用普拉洛芬滴眼液对术后炎症的影响

摘　　要：目的探讨术前预防性使用普拉洛芬滴眼液对年龄相关性白内障患者前房炎症的影响。方法选取2022-06~10月于作者医院就诊并行白内障超声乳化吸除联合人工晶状体植入术的年龄相关性白内障患者,随机分为治疗组(n=24)和对照组(n=25)。对照组术前3日予以左氧氟沙星滴眼液滴眼,治疗组在对照组基础上加用普拉洛芬滴眼液滴眼,比较两组患者术后前房浮游细胞、最佳矫正视力(best corrected visual acuity,BCVA)、角膜水肿、房水中白细胞介素6(interleukin6,IL-6)情况,并进行相关性分析。结果治疗组患者术后3日房水浮游细胞含量低于对照组患者(P<0.05),两组患者术后7日房水浮游细胞、BCVA、角膜水肿程度差异无统计学意义(P>0.05)。治疗组患者与对照组患者房水中IL-6水平差异无统计学意义(P>0.05)。相关性分析显示核级别与术前视力(r=0.789,P<0.01)、术后7日角膜水肿程度(r=0.490,P<0.01)呈正相关,而房水中IL-6含量与术后3日前房细胞(r=-0.063,P>0.05)、术后7日前房细胞(r=0.217,P>0.05)、术后1周角膜水肿(r=0.000,P>0.05)均无相关性。结论年龄相关性白内障患者术前预防性使用普拉洛芬滴眼液不影响房水中IL-6的含量,但能减轻术后早期眼部炎症反应。Objective To investigate the effects of preoperative prophylactic use of pranoprofen eye drops on anterior chamber inflammation in patients with age-related cataracts.Methods Age-related cataract patients who underwent phacoemulsification combined with intraocular lens implantation at the author′s hospital from June to October 2022 were selected and randomly divided into treatment group(n=24)and control group(n=25).The control group received levofloxacin eye drops for 3 days before surgery,while the treatment group received pranoprofen eye drops in addition to levofloxacin eye drops,the postoperative anterior chamber planktonic cells,best corrected visual acuity(BCVA),corneal edema and interleukin 6(IL-6)levels in the aqueous humor were compared between the two groups,and correlation analyses were performed.Results The content of anterior chamber planktonic cells in the treatment group was lower than that in the control group on the 3rd day after surgery(P<0.05),there was no significant difference in anterior chamber planktonic cells,BCVA and degree of corneal edema between the two groups on the 7th day after surgery(P>0.05).There was no significant difference in IL-6 content in the aqueous humor between the treatment group and the control group(P>0.05).Correlation analysis showed a positive correlation between nuclear grade and preoperative visual acuity(r=0.789,P<0.01),as well as the degree of corneal edema on the 7th day after surgery(r=0.490,P<0.01),however,there was no correlation between IL-6 levels in the aqueous humor and anterior chamber cells on the 3rd day after surgery(r=-0.063,P>0.05),anterior chamber planktonic cells on the 7th day after surgery(r=0.217,P>0.05)and corneal edema after one week(r=0.000,P>0.05).Conclusion Prophylactic use of pranoprofen eye drops before surgery in patients with age-related cataracts does not affect the content of IL-6 in the aqueous humor but can alleviate early postoperative ocular inflammation.
