脾俞穴影像解剖及针刺安全深度研究

摘　　要：目的:了解脾俞的影像解剖结构,分析针刺危险深度,明确安全深度,为脾俞的针刺安全提供依据,为临床针刺深度的确定提供参考。方法:用磁共振仪采集219例健康成年志愿者脾俞的横断面图像,分析解剖结构,测量脾俞直刺危险深度,测算安全深度。结果:左右脾俞直刺时的平均危险深度值,男性分别为(4.7±0.8)cm、(4.5±0.8)cm,女性分别为(3.8±0.7)cm、(3.7±0.7)cm。危险深度值与体型的相关性显著,BMI与左、右脾俞穴危险深度值的相关系数分别为0.818、0.811,差异具有统计学意义(P<0.01)。结论:脾俞穴深部为肾,直刺过深可伤及肾。可依据体型确定脾俞的安全针刺深度,肥胖和超重者直刺深度不超过4 cm,体型正常者不超过3 cm,体型过轻者不超过2.5 cm。Objective:To understand the image anatomical characteristics of Pishu(BL20),to analyze the needling dangerous depth,to clarify the safe depth,so as to offer the basis for the safe needling depth of BL20 and to provide the reference for the clinical application of BL20.Methods:The cross-sectional images of 219 healthy adult volunteers of BL20 were collected by magnetic resonance instrument to analyze the anatomical characteristics,to measure the dangerous depth of direct needling BL20,and to calculate the safe depth.Results:The average dangerous depth values of the left and right direct needling BL20 were(4.7±0.8)cm and(4.5±0.8)cm in males,(3.8±0.7)cm and(3.7±0.7)cm in females,respectively.Dangerous depth values were significantly correlated with body size.The related coefficient between body mass index(BMI)and dangerous depth values of the left and right BL20 were 0.818 and 0.811,respectively(P<0.01).Conclusion:The deep part of BL20 is kidney,which can be injured if the puncture is too deep.The safe needling depth of BL20 can be determined by body size,for the obesity and the overweight,the direct needling depth should not exceed 4 cm,for normal weight,it should not exceed 3 cm,and for the underweight person,it should not exceed 2.5 cm.
