摘  要:目的研究计算机三维重建联合3D打印技术在筛窦骨瘤手术中的临床应用效果。方法回顾性研究2014年4月至2020年4月我院收治的28例筛窦骨瘤患者的临床资料,其中14例在治疗过程中采用了计算机三维重建联合3D打印技术(3D打印组),14例采用传统鼻内镜手术切除方法治疗(传统手术组)。比较两组的治疗效果及术后并发症的发生情况。结果3D打印组中所有患者筛窦骨瘤切除均在3D打印实体模型指导下完成,传统手术组采用传统鼻内镜手术完成,手术过程顺利。3D打印组手术时间为(35.3±11.14)min,短于传统手术组的(44.1±10.6)min(P=0.014);3D打印组出血量为(89.6±7.1)mL,少于传统手术组的(105.3±6.1)mL(P=0.027)。28例患者均获得24~45个月随访,平均(28.7±3.2)个月。两组患者术后头痛、鼻塞、面部不适等症状逐渐消失。3D打印组术后出现1例复视,1个月后复视逐渐消失;传统手术组有2例复发,经再次手术后治愈。结论计算机三维重建联合3D打印技术能够准确和直观地反映筛窦骨瘤的立体形态和解剖结构的空间关系,对于筛窦骨瘤的治疗有临床指导作用,和传统鼻内镜手术相比,大大减少了术中出血,缩短了手术时间,能达到满意的手术效果,值得在临床上推广应用。Objective To explore the clinical application effect of digital three-dimensional reconstruction combined with 3D printing technology in the surgery of ethmoid osteomas.Methods A retrospective study was conducted on the clinical data of 28 patients with ethmoid osteomas admitted to our hospital from April 2014 to April 2020.Among them,14 cases were treated with digital three-dimensional reconstruction combined with 3D printing technology(group A),and 14 cases were treated with traditional endoscopic surgery(group B).It was compared of the treatment effectiveness and postoperative complications between the two groups.Results Patients in group A underwent resection of ethmoid osteomas under the guidance of a digital three-dimensional reconstruction model,while group B underwent traditional endoscopic surgery,resulting in a smooth surgical process.The surgical time in group A was(35.3±11.14)minutes,which was shorter than(44.1±10.6)minutes of group B(P=0.014).The bleeding volume in group A was(89.6±7.1)mL,which was less than(105.3±6.1)mL of group B(P=0.027).All 28 patients were followed up for 24~45 months,with an average of(28.7±3.2)months.The symptoms of headache,nasal congestion,and facial discomfort gradually disappeared in both groups after surgery.Diplopia of 1 case occurred postoperatively in group A,and gradually disappeared after one month.There were 2 cases of recurrence in group B,which were cured after reoperation.Conclusion The combination of digital three-dimensional reconstruction and 3D printing technology can accurately and intuitively reflect the spatial relationship of the three-dimensional shape and anatomical structure of ethmoid osteomas,which has clinical guidance for the treatment of ethmoid osteomas.Compared with traditional endoscopic surgery,it greatly reduces intraoperative bleeding,shortens surgical time,and achieves satisfactory surgical results.It is worth promoting and applying in clinical practice.
