摘 要:目的探讨轻、中度骨性Ⅲ类错[牙合]患者Spee曲线深度、下颌体长度、第三磨牙萌出状态与磨牙后间隙之间相关性的研究。方法收集2020年1月~2021年12月来徐州市中心医院口腔正畸科就诊的96例骨性Ⅲ类错[牙合]成年患者的牙齿记存模型、全景片和头颅侧位片,测量每位患者的Spee曲线深度、下颌体长度(Go-Gn)、下颌角角度、第二磨牙与第三磨牙牙长轴交角(α)、磨牙后间隙(RMS)、下牙列拥挤度,根据第三磨牙的萌出情况分为阻生组、萌出组,左右侧的磨牙后间隙、Spee曲线深度及α角采用Wilcoxon符号秩和检验比较;不同性别间磨牙后间隙采用Wilcoxon秩和检验进行比较,阻生组与萌出组间测量数据依据是否满足正态分布分别采用独立样本t检验和Wilcoxon秩和检验进行比较,并将上述测量数据与磨牙后间隙进行Spearman相关性分析,并对筛选出的有意义的因素进行多元线性逐步回归分析。结果骨性Ⅲ类错[牙合]患者阻生组与萌出组间的磨牙后间隙、Go-Gn、下颌角角度、α角、下牙列拥挤度均有统计学差异(P<0.05),阻生组的磨牙后间隙、Go-Gn、下颌角角度均小于萌出组,而α角、下牙列拥挤度大于萌出组,两组间的Spee曲线深度无统计学意义(P>0.05);磨牙后间隙大小与Go-Gn成正相关,与α角、Spee曲线深度成负相关,与下颌角角度、下牙列拥挤度无关。回归分析的结果表明Spee曲线深度、Go-Gn、α角均是磨牙后间隙的影响因素,且Spee曲线的影响程度高于Go-Gn、α角。结论轻、中度骨性Ⅲ类错[牙合]患者的Spee曲线深度、第三磨牙的萌出情况与下颌体长度对下颌磨牙后间隙大小存在影响。Objective To investigate the correlation between Spee curve depth,mandibular body length,thirdmolar eruption status and retromolar space in patients with mild to moderate skeletal classⅢmalocclusion.MethodsThedental model,panoramic radiographs and lateral cephalometric radiographs of 96 patients with skeletal classⅢmalocclusion who visited the Orthodontics Department of Xuzhou Central Hospital from January 2020 to December2021 were collected.According to the eruption of the third molar,the depth of the Spee curve,the length of themandible(Go-Gn),the angle of the mandible,the angle between the long axis of the second molar and the thirdmolar(α),and the retromolar space(RMS)and the lower dental arch crowding of the patient were divided intothe impacted group and the eruption group.The left and right retromolar space,Spee curve depth andαangle werecompared by Wilcoxon signed rank sum test.The retromolar space between different genders was compared byWilcoxon rank sum test.The measured data between the impacted group and the eruption group were compared byindependent samples t test and Wilcoxon rank sum test according to whether they met the normal distribution,and Spearman correlation analysis was performed between the above measured data and the retromolar space,andperform multiple linear stepwise regression analysis on the screened meaningful factors.ResultsThere weresignificant differences in the retromolar space,Go-Gn,mandibular angle,αangle and lower dental arch crowding betweenthe impacted group and the erupted group(P<0.05).The retromolar space,Go-Gn and mandibular angle in the impactedgroup were lower than those in the erupted group,but theαangle and lower dental arch crowding was larger than that in the erupted group.There was no significant difference in thedepth of Spee curve between the two groups(P>0.05).The size of retromolar space was positively correlated with Go-Gn and negatively correlated withαangle and Spee curvedepth,but not with mandibular angle and lower dental archcrowding.The result
