摘 要:目的探讨颈部核心稳定训练联合稳定咬合板治疗TMD的疗效。方法选取符合TMD诊断标准的患者212例,年龄18~60周岁,采用区组随机法分为对照组和治疗组各106例。对照组给予常规理疗+颈部核心稳定训练治疗,治疗组在对照组基础上增加佩戴稳定咬合板。在治疗前、治疗后及治疗后3个月时评估所有患者的疼痛视觉模拟评分(VAS)、最大张口度(MMO)及Fricton颞下颌关节紊乱指数,在治疗后及治疗后3个月记录所有患者的总体症状改变等级评分(GRCS)。结果组间比较显示,2组治疗后及治疗后3个月Fricton指数、MMO、VAS评分比较差异均有统计学意义(P<0.05),NDI比较差异无统计学意义(P>0.05)。组内比较显示,2组患者治疗后及治疗后3个月Fricton指数、VAS评分、MMO、NDI比较差异有统计学意义(P<0.05)。GRCS评分结果显示,2组治疗后及治疗后3个月比较差异有统计学意义(P<0.05)。结论颈部核心稳定训练联合稳定咬合板治疗能够更好的缓解TMD疼痛,改善颞下颌关节功能和开口度。Objective To investigate the effect of neck core stabilization training and stabilization splint in thetreatment of temporomandibular joint disorders.Methods212 patients with TMD,aged 18-60,were randomlydivided into the control group and the treatment group with 106 cases each.The control group was treated withphysiotherapy,neck core stability training.The treatment group was added with stabilization splint.The visualanalogue score(VAS),maximum mouth opening(MMO)and Fricton temporomandibular joint disorder index of allpatients were evaluated before treatment,after treatment and 3 months following,and the Global Rating of ChangeScales(GRCS)of all patients was recorded after treatment and 3 months following.ResultsThe comparisonbetween the two groups showed that there were significant differences in Fricton index,MMO,VAS scoresbetween the two groups after treatment and 3 months following(P<0.05),while there was no significant differencein NDI(P>0.05).The intra group comparison showed that there were significant differences in Fricton index,VASscore,MMO,NDI between the two groups after treatment and 3 months following(P<0.05).GRCS score showedthat there was a statistically significant difference between the two groups after treatment and 3 months following(P<0.05).ConclusionCompared with neck core stability training,neck core stability training combined withstabilization splint relieve pain better,improve temporomandibular joint function and opening degree,and thewearing of stabilization splint compensated for the unstable effect of neck core stability training on TMD.
