50例舌癌术后舌功能评价

摘　　要：目的探讨舌癌患者术后舌功能评价。方法选取50例行舌癌切除术并同期行舌体修复患者,将其根据Urken分类分为四组,分别收集患者术后吞咽功能,语音功能及舌活动度等进行分析,比较不同修复方式术后舌的形态与功能。结果Ⅰ类舌体缺损患者术后功能相比Ⅱ类舌缺损患者恢复较好;Ⅱ类舌体缺损患者组内前臂游离皮瓣修复患者比股前外侧游离皮瓣修复患者功能恢复较好。结论舌体缺损较小的患者,局部拉拢缝合能在最大程度上恢复其术后功能;舌体缺损1/3~1/2者,前臂游离皮瓣相对于股前外侧游离皮瓣更有优势;对于舌体缺损超过中线或者舌根缺损的患者,使用股前外侧游离皮瓣,能有效还原舌体体积和形态,获得更好的语音及吞咽功能效果。Objective To evaluate the tongue function in patients with tongue cancer.Methods Fifty patients who underwent surgery were divided into four groups according to the Urken classification.The swallowing function,speech function and tongue mobility of the patients were collected and analyzed to compare the morphology and function of the tongue after tongue surgery with different repair methods.Results The functional recovery of patients with classⅠtongue defects was significantly better than that of patients with classⅡtongue defects.In the group of patients with classⅡtongue defects,the functional recovery of patients repaired with forearm free flap was better than that of patients repaired with anterolateral thigh free flap.Conclusion For patients with small tongue defects,local closure and suturing can restore the postoperative function to the greatest extent.Forearm free flap has more advantages than anterolateral thigh free flap in patients with tongue defects of 1/3~1/2.For patients with tongue defects beyond the midline or tongue base defects,the use of free anterolateral thigh flap can effectively restore the volume and shape of tongue body,and obtain better speech and swallowing function.
