摘　　要：目的研究不同根管冲洗方案对不可复性牙髓炎根管治疗术后患者疼痛及预后的影响。方法选取2019年1月-2022年3月万载县人民医院收治的72例不可复性牙髓炎患者,按照随机数字表法分为对照组(36例)与观察组(36例)。两组均给予根管治疗,对照组应用注射根管冲洗方案,观察组则应用超声根管冲洗方案,比较两组根管充填质量(根充致密性、侧支根管充填率)、牙周指数[菌斑指数(PLI)、探诊出血指数(BI)、牙龈指数(GI)]、疼痛视觉模拟评分(VAS)、口腔健康程度量表(OHIP-14)评分、并发症发生率。结果观察组根充致密性优于对照组,侧支根管充填率高于对照组(P<0.05);两组术后3个月PLI、BI、GI均小于术后即刻,且观察组小于对照组(P<0.05);两组术后24 h VAS评分低于术后6 h,且观察组低于对照组(P<0.05);两组术后3个月OHIP-14评分低于术后1周,且观察组低于对照组(P<0.05);观察组并发症发生率为8.33%,低于对照组的27.78%(P<0.05)。结论超声根管冲洗方案与注射根管冲洗方案在不可复性牙髓炎根管治疗术中均具有确切应用效果,前者可改善根管充填质量,且术后疼痛轻、并发症少,对患者口腔健康具有积极改善作用,其效果优于注射根管冲洗方案。Objective To study the effect of different root canal irrigation schemes on pain and prognosis of patients with irreversible pulpitis after root canal therapy.Methods A total of 72 patients with irreversible pulpitis admitted to Wanzai County People's Hospital from January 2019 to March 2022 were selected and divided into control group(36 patients)and observation group(36 patients)according to the random number table method.Both groups were given root canal treatment,while the control group was treated with injection root canal irrigation scheme,the observation group was treated with ultrasonic root canal irrigation scheme.The root canal filling quality(root filling compactness,collateral root canal filling rate),periodontal index[plaque index(PLI),bleeding index(BI),gingival index(GI)],visual analogue scale(VAS),oral health scale(OHIP-14)score and complication rate were compared between the two groups.Results The root filling density of the observation group was better than that of the control group,and the lateral root canal filling rate was higher than that of the control group(P<0.05).The PLI,BI and GI of the two groups at 3 months after operation were smaller than those immediately after operation,and those of the observation group were smaller than those of the control group(P<0.05).The VAS score of the two groups at 24 h after operation was lower than that at 6 h after operation,and that of the observation group was lower than that of the control group(P<0.05).The OHIP-14 score of the two groups at 3 months after operation was lower than that at 1 week after operation,and that of the observation group was lower than that of the control group(P<0.05).The incidence of complications in the observation group was 8.33%,which was lower than 27.78%in the control group(P<0.05).Conclusion The ultrasonic root canal irrigation scheme and the injection root canal irrigation scheme have a definite application effect in the root canal therapy of irreducible pulpitis.The former can improve the quality of root canal filli
