基于IKAP模式的康复指导与徒手淋巴引流对于乳腺癌术后上肢淋巴水肿与功能康复的影响

摘　　要：目的 分析基于信息-知识-信念-行为(IKAP)模式的康复指导与徒手淋巴引流对于乳腺癌术后上肢淋巴水肿与功能康复的影响。方法 选取2021年5月至2023年4月期间于本院接受改良乳腺癌根治术的患者60例,使用随机数字表法进行分组。对照组30例患者采取常规护理措施,研究组30例患者在常规护理的基础上联合基于IKAP模式的康复指导与徒手淋巴引流进行护理。针对两组患者的上肢淋巴水肿发生率、肩关节活动度、Fugl-Meyer量表(上肢部分)评分、生命质量评定测定量表(FACT-B)评分,以及其他术后并发症情况进行比较。结果 研究组上肢淋巴水肿的发生率低于对照组(P <0.05)。干预2个月后,研究组肩关节前屈、内收、后伸、外展角度大于对照组(P <0.05),Fugl-Meyer量表(上肢部分)评分高于对照组(P <0.05),FACT-B量表分值低于对照组(P <0.05)。其他术后并发症比较中,研究组结果较对照组低(P <0.05)。结论 基于IKAP模式的康复指导联合徒手淋巴引流可以有效预防乳腺癌术后上肢淋巴水肿,改善患肢肩关节活动角度与上肢功能,降低并发症风险,提高患者生活质量。Objective To analyze the effect of rehabilitation guidance based on information knowledge belief behavior(IKAP) model and freehand lymphatic drainage on upper limb lymphedema and functional rehabilitation after breast cancer surgery.Methods 60 patients with breast cancer who received modified radical mastectomy in our hospital from May 2021 to April 2023 were selected and grouped by random number table.The control group consisted of 30 patients who received routine nursing measures,while the study group consisted of 30 patients who received rehabilitation guidance based on the IKAP model and freehand lymphatic drainage on the basis of routine nursing.Compare the upper limb lymphedema,shoulder joint range of motion,Fugl Meyer scale(upper limb part) score,quality of life assessment scale(FACT-B) score,and other postoperative complications between two groups of patients.Results The incidence of upper limb lymphedema in the study group was lower than that in the control group(P<0.05).After 2 months of intervention,the angles of flexion,adduction,extension,and abduction in the study group were greater than those in the control group(P<0.05),the Fugl Meyer scale(upper limb part) score of the study group was higher than that of the control group(P<0.05),and the FACT-B scale score was lower than that of the control group(P<0.05).Among other postoperative complications,the results of the study group were lower than those of the control group(P<0.05).Conclusions Rehabilitation guidance based on IKAP mode combined with freehand lymphatic drainage can effectively reduce the incidence of upper limb lymphedema after breast cancer surgery,improve shoulder joint movement angle and upper limb function,reduce the risk of complications,and improve the quality of life.
