腹腔镜子宫病损切除术对治疗子宫平滑肌瘤的临床应用效果

摘　　要：目的 探究腹腔镜子宫病损切除术对治疗子宫平滑肌瘤的临床应用效果。方法 于2020年1月至2022年12月期间,从我院妇科收治的子宫平滑肌瘤患者中选择60例为研究对象,其中2020年1月至2021年5月期间的30例患者为试验A组,行腹腔镜子宫全切术,2021年6月至2022年12月期间的30例患者为试验B组,行腹腔镜子宫病损切除术,两组患者均符合子宫平滑肌瘤切除的手术指征。记录并比较两组患者的手术后并发症发生率、手术情况、术前及术后性激素水平、肌瘤复发率、术前及术后的性功能评分、炎性因子水平、免疫功能。结果 试验B组患者的手术后并发症发生率、手术情况、术前及术后性激素水平、肌瘤复发率、术前及术后的性功能评分、炎性因子水平、免疫功能等指标均显著优于试验A组,P <0.05,存在统计学差异。结论 采用腹腔镜子宫病损切除术对子宫平滑肌瘤患者进行治疗,不仅可有效改善合并缺铁性贫血患者的贫血症状,提高其手术成功率,还可大大减少术后恢复时间,降低术中出血量,缩短住院时间。与腹腔镜子宫全切术相比,腹腔镜子宫病损切除术不仅可使患者手术耐受性良好,还能有效降低患者术后切口感染、尿潴留等术后并发症的发生率,且其术后创伤小,费用少,术后性激素水平稳定。Objective To explore the clinical application effect of laparoscopic hysterectomy for the treatment of uterine leiomyoma.Methods Clinical data of 60 patients with uterine leiomyoma who underwent total laparoscopic hysterectomy or laparoscopic hysterectomy for lesions of uterus admitted to Department of Gynecology of our hospital from January 2020 to December 2022 were retrospectively analyzed.Among them,30 patients who underwent laparoscopic total hysterectomy were selected as group A,the remaining 30 patients who underwent laparoscopic hysterectomy were included in group B.Postoperative complication rate,operation related indexes,sex hormone level before and after operation,fibroid recurrence rate,sexual function score before and after operation,inflammatory factor level and immune function were compared between the two groups.Results The postoperative complication rate,operation related indexes,postoperative sex hormone level,postoperative sexual function score and immune function of group B were better than those of group A,and the fibroid recurrence rate and inflammatory factor levels were lower than those of group A(all P<0.05).Conclusions Compared with total laparoscopic hysterectomy in treatment of uterine leiomyoma,laparoscopic hysterectomy for lesions can significantly reduce intraoperative bleeding and postoperative complications,better promote postoperative rehabilitation and shorten hospital stay.
