摘  要:目的研究一种新型泪液分泌功能检测方法———快速检测泪河试纸(strip meniscometry tube,SMT)检测与临床常用干眼诊断参数间的相关性及其对干眼的诊断价值。方法采用回顾性病例研究方法,收集眼科门诊干眼患者的OSDI(ocular surface disease index)问卷调查、FSBUT(fluorescein stained tear film breakup time)、CFS(corneal fluorescein staining score)、SMT及SⅠT(Schirmer’s testⅠ)检测结果。采用Spearman相关性分析、组内相关系数、两独立样本秩和检验及ROC曲线等检验方法对数据进行处理。结果共25名患者(25眼)完成研究,患者年龄(44.28±11.74)岁,年龄范围为21~62岁。25眼的OSDI平均值为(62.32±10.97),FSBUT为(1.20±1.12)s,CFS为(2.04±1.86);SMT为(2.32±1.11)mm/5s;SⅠT为(7.56±7.25)mm/5min。相关性分析结果示:SMT与OSDI、FSBUT、CFS及SⅠT之间均具有相关性(P=0.024、P=0.043、P=0.043及P=0.040),SⅠT与OSDI、CFS及SMT具有相关性(P=0.020、P=0.026及P=0.040),提示SMT较SⅠT与临床常用干眼诊断参数的总体相关性更佳。重复性分析结果示:SMT和SⅠT的ICC值分别为0.806和0.555,SMT的ICC>0.75,而SⅠT的ICC<0.75,表明SMT重复性良好,优于SⅠT。分析不同严重程度的干眼SMT测量值示:轻中度和重度干眼的SMT平均值分别为(2.73±1.27)mm/5s和(1.86±0.66)mm/5s,平均SMT均<4mm/5s提示两类患者的SMT均异常,重度干眼的SMT显著低于轻中度干眼(P=0.040)。结论SMT作为一种便捷、舒适的泪液分泌功能检测方法,与临床常用干眼诊断参数间的相关性较好,重复性佳,具有较好的临床应用前景。Objective To investigate diagnostic value of a new method of tear secretion function,strip meniscometry tube(SMT),and its correlation with common clinical diagnostic parameters of dry eye.Methods Retrospective case study was used.OSDI(Ocular surface disease index)questionnaire,FSBUT(fluorescein stained tear film breakup time),CFS(corneal fluorescein staining score),SMT and SⅠT(Schirmer’s TestⅠ)results of dry eye patients in ophthalmology outpatient department were collected.Spearman correlation analysis,intra-group correlation coefficient,rank sum test of two independent samples and ROC curve were used for data processing.Results A total of 25patients(25eyes),with an average age of(44.28±11.74)years,completed the study.The mean OSDI score of 25eyes was(62.32±10.97),FSBUT was(1.20±1.12)s,mean CFS was(2.04±1.86).Mean SMT was(2.32±1.11)mm/5s,mean SⅠT was(7.56±7.25)mm/5min.Correlation analysis results showed that SMT was correlated with OSDI,FSBUT,CFS and SⅠT(P=0.024,P=0.043,P=0.043and P=0.040).SⅠT was correlated with OSDI,CFS and SMT(P=0.020,P=0.026and P=0.040),suggesting that SMT had a better overall correlation than SⅠT with common clinical diagnostic parameters.The repeatability analysis show that the ICC values of SMT and SⅠT were 0.806and 0.555respectively,and the ICC values of SMT were>0.75while those of SⅠT were<0.75,indicating that the measurement consistency of SⅠT was poorer than that of SMT.Analysis of SMT measurements in different degrees of dry eye shows:the mean SMT values of mild to moderate dry eye and severe dry eye were(2.73±1.27)mm/5sand(1.86±0.66)mm/5s,respectively.The mean SMT values were lower than 4mm/5s,indicating that SMT values were abnormal in both groups.The SMT value of severe dry eye was significantly lower than that of mild to moderate dry eye(P=0.040).Conclusion SMT,as a convenient and comfortable detection method for tear secretion function,has a strong correlation with common clinical diagnostic parameters of dry eye,and has satisfying clinical applicatio
