摘　　要：目的探讨24h动态脑电图(24hAEEG)检查在癫痫所致精神障碍患者诊治中的应用价值。方法选取186例我院2018年4月~2020年4月就诊的癫痫所致精神障碍患者,根据检查方式不同分为三组[24hAEEG组62例、常规脑电图(REEG)组62例、视频脑电图(VEEG)组62例]。统计比较三组脑电图检查结果、异常波检出率、临床疗效及1年后随访检查结果与疗效关系。结果与REEG、VEEG检查相比,24hAEEG检查总异常率及慢波、慢波暴发、发作波检出率较高(P<0.05);与REEG、VEEG组相比,24hAEEG组临床无发作结果正常比例较低,临床无发作结果无改善比例较高(P<0.05)。结论癫痫所致精神障碍患者采用24hAEEG检查可有效提高对异常脑电波的检出率,定期采用24hAEEG检查可作为其评估疗效的可靠依据,对临床进行诊断提供有效方案。Objective To investigate the application value of 24h ambulatory electroencephalography(24hAEEG)in the diagnosis and treatment of patients with epilepsy-induced mental disorders.Methods A total of 186 patients with mental disorders caused by epilepsy who were treated in our hospital from April 2018 to April 2020 were selected and divided into three groups according to different examination methods[24hAEEG group 62 cases,routine electroencephalography(REEG)group 62 cases,video Electroencephalography(VEEG)group of 62 cases].The EEG examination results,abnormal wave detection rate,clinical efficacy,and the relationship between the results of fllow-up examinations after 1 year and the efficacy were statistically compared among the three groups.Results Compared with REEG and VEEG,the total abnormal rate,slow wave,slow wave burst,and seizure wave detection rate of 24hAEEG were higher(P<0.05).The proportion was lower,and the proportion of no improvement in clinical seizure-free results was higher(P<0.05).Conclusion 24hAEEG examination can effectively improve the detection rate of abnormal brain waves in patients with mental disorders caused by epilepsy.Regular 24hAEEG examination can be used as a reliable basis for evaluating the curative effect and provide an effective solution for clinical diagnosis.
