双歧杆菌四联活菌片联合雷贝拉唑四联疗法治疗老年幽门螺杆菌(Hp)感染阳性胃溃疡的效果

摘　　要：目的研究双歧杆菌四联活菌片联合雷贝拉唑四联疗法治疗老年幽门螺杆菌(Hp)感染阳性胃溃疡的临床效果。方法选取2019年6月~2020年6月我院老年Hp感染阳性胃溃疡患者98例,按照随机数字表法分为常规组、研究组,各49例。常规组采用雷贝拉唑四联疗法,研究组在常规组基础上采用双歧杆菌四联活菌片治疗。比较两组疗效、Hp根除率、临床症状评分、胃肠激素[SS(生长抑素)、MTL(胃动素)]、肠道菌群(双歧杆菌、肠球菌、酵母菌)及不良反应。结果研究组总有效率95.82%、Hp根除率87.76%高于常规组79.59%、71.43%(P<0.05);治疗4周后研究组临床症状评分低于常规组(P<0.05);治疗4周后研究组MTL水平低于常规组,SS水平高于常规组(P<0.05);治疗4周后研究组双歧杆菌、肠球菌高于常规组,酵母菌低于常规组(P<0.05);两组不良反应发生率比较,无统计学差异(P>0.05)。结论双歧杆菌四联活菌片联合雷贝拉唑四联疗法治疗老年Hp感染阳性胃溃疡疗效显著,能提高Hp根除率,改善临床症状,调节胃肠激素水平与肠道菌群,安全性高。Objective To study the clinical effect of bifidobacteria quadruple viable tablets combined with rabeprazole quadruple therapy in the treatment of elderly patients with Helicobacter pylori(Hp)positive gastric ulcer.Methods From June 2019 to June 2020,98 elderly patients with Hp infection-positive gastric ulcer in our hospital were selected,and they were divided into a routine group and a study group according to the random number table method,with 49 cases in each group.The conventional group was treated with rabeprazole quadruple therapy,and the research group was treated with bifidobacteria quadruple viable tablets on the basis of the conventional group.Compare the two groups'efficacy,Hp eradica-tion rate,clinical symptom score,gastrointestinal hormones[SS(Somatostatin),MTL(Motilin)],intestinal flora(Bifidobac-terium,Enterococcus,yeast)and adverse reactions.Results The total effective rate of the study group was 95.82%and the Hp eradication rate of 87.76%was higher than that of the conventional group 79.59%and 71.43%(P<0.05);the clini-cal symptom score of the study group was lower than that of the conventional group(P<0.05)after 4 weeks of treatment;After 4 weeks,the MTL level of the study group was lower than that of the conventional group,and the SS level was higher than that of the conventional group(P<0.05);after 4 weeks of treatment,the bifidobacteria and enterococci of the study group were higher than those of the conventional group,and the yeasts were lower than the conventional group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Bifidobacterium quadruple viable tablets combined with rabeprazole quadruple therapy has a significant effect on the treat-ment of Hp infection-positive gastric ulcers in the elderly.It can improve Hp eradication rate,improve clinical symptoms,regulate gastrointestinal hormone levels and intestinal flora,and is safe High sex.
