卞嵩京运用清热利湿、行气化瘀法治疗膀胱癌经验介绍

摘　　要：介绍卞嵩京教授运用清热利湿、行气化瘀法治疗膀胱癌的临床经验。卞嵩京教授认为,膀胱癌初起脾肾两虚,膀胱气化不利,继而湿、热、瘀蕴结下焦为患,久则蓄积为毒而发病,病机总属下焦湿热瘀毒、膀胱气化不利,故总的治疗法则为清热利湿、化瘀解毒、宣畅气机。常用八正散、当归贝母苦参丸合白术木香散加减,名为鱼龙通淋汤。临证强调谨守病机,守方不变;稍改处方,以应变证。处方用药兼容并蓄,经方、古方可新用,时方、草药宜选用。若能识得根本,选用达药,方可效如桴鼓。This paper introduces the clinical experience of Professor BIAN Songjing applying methods of clearing heat and draining dampness and moving qi and dissolving stasis in the treatment of bladder cancer.Professor BIAN holds that bladder cancer starts with the spleen-kidney deficiency and the disturbance of qi transformation in the bladder,then the accumulation of dampness,heat,and blood stasis in the lower energizer becomes the cause and turns into toxin after a long time,leading to the onset of the disease.The main pathogenesis is damp-heat,stasis and toxin in the lower energizer and the disturbance of qi transformation in the bladder therefore the general treatment principle is to clear heat and drain dampness,dissolve blood stasis and resolve toxins,and promote qi movement.Bazheng Powder,Danggui Beimu Kushen Pills,and Baizhu Muxiang Powder are often combined together with modification,being known as Yulong Tonglin Decoction.The clinical treatment should always be in accordance with pathogenesis and adhere to the formulas;there can be small adjustments to the prescription for any changes in the syndrome.The prescription and medication are inclusive;both formulas and ancient prescriptions can be used in a new way,and current prescriptions and Chinese medicinals should be used with proper selection.It will work like a charm if one can recognize the root of the disease and choose medicines with reliable effectiveness.
