顺行单克氏针髓内固定治疗青少年第五掌骨颈骨折的临床效果

摘　　要：目的:观察顺行克氏针髓内固定术治疗青少年第五掌骨颈骨折(FMNF)的效果。方法:选择2018年9月—2020年5月浙江省宁波市鄞州浙东骨科医院收治的FMNF患者32例作为研究对象,依据治疗方式不同分为对照组与研究组,各16例。对照组应用常规非手术治疗,研究组应用顺行单克氏针髓内固定治疗。比较两组患者术后随访情况、骨折愈合情况、运动范围及双侧上肢功能。结果:研究组患者术后随访复查X线片显示均复位愈合,愈合时间为(2.61±0.63)个月。对照组术后随访复查X线片显示2例患者20°~30°轻度成角愈合,14例患者无移位,愈合时间为(5.41±1.21)个月。术后12个月,研究组向掌侧成角、上肢功能评定表评分低于对照组,运动范围大于对照组,差异有统计学意义(P<0.05)。结论:顺行单克氏针髓内固定治疗青少年FMNF可促进骨折愈合,有助于恢复指关节活动范围,改善上肢功能。Objective:To observe the therapeutic effect of intramedullary fixation with a parallel Kirschner pin in the treatment of fifth metacarpal neck fracture(FMNF)in adolescents.Methods:32 patients with FMNF admitted to Zhejiang Ningbo Yinzhou Zhe⁃dong Orthopaedic Hospital from September 2018 to May 2020 were selected as the study subjects,and they were divided into the control group and the study group based on the individual therapeutic demand.Each group with 16 cases.The control group was treated with conventional non-surgical treatment,and the study group was treated with intramedullary fixation with a single Kirsch⁃ner pin in a smooth manner.Postoperative follow-up,fracture healing,range of motion and bilateral upper limb function were com⁃pared between the two groups.Results:The postoperative follow-up X-rays of the patients in the study group showed that all the fractures were in place and healed,and the healing duration was(2.61±0.63)months.In the control group,postoperative follow-up radiographs showed mild angular healing at 20°-30°in 2 patients and no displacement in 14 patients,with a healing time of(5.41±1.21)months.Twelve months after the operation,the study group had lower scores of angle formation toward the palmar side and upper extremity function rating scale than the control group,and the range of motion was greater than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:The treatment of adolescents with FMNF by smooth single Kirschner pin intramedullary fixation can promote fracture healing,help to restore the range of motion of the phalangeal joints,and improve the function of the upper limbs.
