摘  要:目的 探究固定矫治器联合微种植支抗垂直控制对上颌前突错畸形患者软硬组织和颏部形态变化的影响。方法 选取2020年3月至2022年3月铜川矿务局中心医院收治的80例上颌前突错畸形患者,依据随机数表法分为对照组及观察组,每组40例。对照组患者予以直丝弓矫治器治疗,观察组患者予以固定矫治器联合微种植支抗垂直控制治疗。比较两组患者治疗前、治疗4周后上颌相对于颅部的前后位置关系(SNA)、下颌相对于颅部的前后位置关系(SNB)、反映上下颌骨对颅部的相互位置关系(ANB)、上中切牙的倾斜度和突度距离(U1-NA)、下中切牙长轴与鼻根点-下齿槽座点的交角(L1-NB)、上下前部牙弓的突度(UL-U1)、下唇突点至下中切牙唇面的距离(LL-L1)、上切牙切缘到下切牙唇面的水平距离(Overjet)、上前牙覆盖过下前牙唇面的垂直距离(Overbite)、上唇突度,上唇突点至E线的垂直距离(E-upper-lip)、下唇突度,下唇突点至E线的垂直距离(E-lower-lip)、鼻唇角、颏沟倾角、颏软组织厚度、颏部形态、咀嚼功能及垂直向量参数。结果 治疗后,两组患者的UI-NA、UI-NA、LI-NB、LI-NB、Overjet、Overbite、E-upper-lip、E-lower-lip显著降低,LL-L1、鼻唇角、颏沟倾角及颏软组织厚度显著升高,且观察组患者的UI-NA、UI-NA、LI-NB、LI-NB、Overjet、Overbite、E-upper-lip、E-lower-lip明显低于对照组,LL-L1、鼻唇角、颏沟倾角及颏软组织厚度明显高于对照组,差异均有统计学意义(P<0.05);治疗后,两组患者的颏高度、颏突度、颏曲度显著升高,根尖点颏厚度、下齿槽座点颏厚度、颏前点厚度、颏凹陷、颏倾斜度显著降低,且观察组患者的颏高度、颏突度、颏曲度明显高于对照组,根尖点颏厚度、下齿槽座点颏厚度、颏前点厚度、颏凹陷、颏倾斜度明显低于对照组,差异均有统计学意义(P<0.05);治疗后,两组患者的咬合力及咀嚼效率�Objective To investigate the effects of fixed appliance combined with mini-implant anchorage for vertical control on soft and hard tissues and morphological changes of chin in patients with maxillary protrusion and mal-occlusion.Methods A total of 80 patients with maxillary protrusion and malocclusion who were admitted to Tongchuan Mining Bureau Central Hospital from March 2020 to March 2022 were selected.The patients were divided into a control group and an observation group by random number table method,with 40 patients in each group.The patients in the control group were treated with straight wire appliance,while the patients in the observation group were treated with fixed appliance combined with mini-implant anchorage for vertical control.The sella-nasion-A point(SNA),sella-nasi-on-B point(SNB),ANB,U1-NA,L1-NB,UL-U1,LL-L1,Overjet,Overbite,upper lip protrusion,E-upper-lip,lower lip protrusion,E-lower-lip,nasolabial angle,chin sulcus inclination,thickness of chin soft tissue,chin morphology,mastica-tory function,and vertical vector parameters before treatment and after 4 weeks of treatment were compared between the two groups.Results After treatment,the UI-NA,UI-NA,LI-NB,LI-NB,Overjet,Overbite,E-upper-lip,and E-low-er-lip of the two groups were significantly decreased,while LL-L1,nasolabial angle,chin sulcus inclination,and thick-ness of chin soft tissue were significantly increased;the UI-NA,UI-NA,LI-NB,LI-NB,Overjet,Overbite,E-upper-lip,and E-lower-lip of the observation group were significantly lower than those of the control group,and LL-L1,nasolabial angle,chin sulcus inclination,and thickness of chin soft tissue were significantly higher than those in the control group;the differences were statistically significant(P<0.05).After treatment,the chin height,chin protrusion,and chin curva-ture of the two groups significantly increased,while the apical chin thickness,inferior alveolar seat chin thickness,anteri-or chin thickness,chin depression,and chin inclination significantly decreased;the chin height,chin
