MicroRNA-218、microRNA-193b在宫颈癌组织中的表达及与化疗耐药性的关系

摘　　要：目的 探讨microRNA-218(miR-218)、microRNA-193b(miR-193b)在宫颈癌组织中的表达及其与化疗耐药性的关系。方法 选取2018年1月-2023年1月成都医学院第一附属医院收治的101例行手术治疗的宫颈癌患者。患者均行紫杉醇、顺铂、阿霉素、5-氟尿嘧啶、卡铂药物敏感性试验,采用实时荧光定量聚合酶链反应检测miR-218、miR-193b表达情况。比较癌组织、癌旁组织中miR-218、miR-193b表达情况,统计化疗药物耐药情况,分析宫颈癌组织中miR-218、miR-193b的表达与化疗耐药性的关系。结果 癌组织中miR-218相对表达量低于癌旁组织(P <0.05),miR-193b相对表达量高于癌旁组织(P <0.05)。宫颈癌患者行体外化疗药物紫杉醇、顺铂、阿霉素、5-氟尿嘧啶、卡铂的耐药率分别为38.61%、46.53%、48.51%、26.73%和40.59%。宫颈鳞癌与宫颈腺癌患者对体外化疗药物紫杉醇、顺铂、阿霉素、5-氟尿嘧啶、卡铂耐药率比较,差异均无统计学意义(P>0.05)。耐药患者癌组织中miR-218相对表达量均低于敏感患者,miR-193b相对表达量均高于敏感患者(P <0.05)。受试者工作特征曲线分析结果显示,miR-218、miR-193b及两者联合预测宫颈癌患者化疗耐药性的敏感性分别为74.51%(95%CI:0.601,0.852)、70.59%(95%CI:0.560,0.821)、82.35%(95%CI:0.686,0.911),特异性分别为74.00%(95%CI:0.594,0.849)、78.00%(95%CI:0.637,0.880)、90.00%(95%CI:0.774,0.963),曲线下面积分别为0.754(95%CI:0.661,0.847)、0.743(95%CI:0.643,0.843)、0.890(95%CI:0.824,0.956)。结论 宫颈癌组织中miR-218表达低于癌旁组织、miR-193b表达高于癌旁组织,miR-218、miR-193b的表达水平与宫颈癌患者化疗耐药性有关,两者联合预测宫颈癌患者化疗耐药性效能良好。Objective To investigate the expressions of miR-218 and miR-193b in cervical cancer tissues and their relationship with chemotherapy resistance.Methods A total of 101 patients with cervical cancer admitted to our hospital from January 2018 to January 2023 for surgical treatment were selected.All patients received drug sensitivity tests for paclitaxel,cisplatin,doxorubicin,5-fluorouracil and carboplatin.The expressions of miR-218 and miR-193b were detected by quantitative real-time polymerase chain reaction and were compared in cancer tissues and adjacent tissues.The resistance to chemotherapy was recorded,and the relationship between the expressions of miR-218 and miR-193b in cervical cancer tissues and chemotherapy resistance was analyzed.Results The relative expression of miR-218 in cancer tissues was lower than that in adjacent tissues(P<0.05),and the relative expression level of miR-193b was higher than that in adjacent tissues(P<0.05).In patients with cervical cancer,the drug resistance rates of paclitaxel,cisplatin,doxorubicin,5-fluorouracil and carboplatin were 38.61%,46.53%,48.51%,26.73%and 40.59%,respectively.There was no significant difference in the drug resistance rates of paclitaxel,cisplatin,doxorubicin,5-fluorouracil and carboplatin between patients with squamous cell carcinoma and adenocarcinoma(P>0.05).The relative expression of miR-218 was lower and that of miR-193b was higher in patients with drug-resistant cervical cancer compared with those with drug-sensitive cervical cancer(P<0.05).Receiver operating characteristic(ROC)curve analysis revealed that the sensitivities of miR-218,miR-193b and their combination for predicting chemotherapy resistance in patients with cervical cancer were 74.51%(95%CI:60.09%,85.23%),70.59%(95%CI:55.98%,82.09%),and 82.35%(95%CI:68.64%,91.13%),with the specificities being 74.00%(95%CI:59.39%,84.92%),78.00%(95%CI:63.67%,88.01%),and 90.00%(95%CI:77.41%,96.26%),and the areas under the curves being 0.754(95%CI:0.661,0.847),0.743(95%CI:0.643,0.843)and 0.890(95%CI:0.824,0
