摘 要:目的探讨初乳口腔涂抹在早产儿中的应用效果。方法选取2019年1月—2021年11月萍乡市人民医院新生儿科诊治的50例早产儿作为研究对象,采用随机数字表将其分为对照组(25例)与观察组(25例)。对照组未予初乳口腔涂抹,观察组给予初乳口腔涂抹。对比两组出生第1天、第7天的血清学相关指标[分泌型免疫球蛋白A(SIgA)、乳铁蛋白(LTF)]与7 d内感染发生率。结果两组早产儿出生第1天的SIgA、LTF水平相比,差异无统计学意义(P>0.05);对照组早产儿出生第7天的SIgA、LTF水平低于出生第1天,观察组早产儿出生第7天的SIgA、LTF水平高于出生第1天,且观察组早产儿出生第7天的SIgA、LTF[(11.48±1.41)μg/mL、(1070.90±90.29)μg/mL]水平均高于对照组[(8.77±0.79)μg/mL、(844.24±87.90)μg/mL],差异有统计学意义(P<0.05);出生7 d内,观察组感染发生率[16.00%(4/25)]与对照组[40.00%(10/25)]比较,差异无统计学意义(P>0.05)。结论初乳口腔涂抹能够有效提高早产儿机体的SIgA、LTF水平,使早产儿获得良好的营养支持与免疫保护,减少感染发生率。Objective To explore the application effect of oral smear of colostrum on premature infants.Methods 50 premature infants treated in the Department of Neonatology of Pingxiang People's Hospital from January 2019 to November 2021 were selected as research objects,and they were divided into control group(25 cases)and observation group(25 cases)by random number table.The control group were not given oral smear of colostrum,while the observation group were given oral smear of colostrum.Serum related indexes(secretory immunoglobulin A[SIgA],lactoferrin[LTF])on the 1st and 7th day of birth and infection rate within 7 days after birth were compared between the two groups.Results There was no statistically significant difference in SIgA and LTF levels between the two groups on the 1st day after birth(P>0.05).On the 7th day of birth,the levels of SIgA and LTF of premature in the control group were lower than those on the first day,while those in the observation group were higher than those in the first day,and the levels of SIgA and LTF([11.48±1.41]μg/mL,[1070.90±90.29]μg/mL)in the observation group were higher than those in the control group([8.77±0.79]μg/mL,[844.24±87.90]μg/mL),and differences were statistically significant(P<0.05).Within 7 days after birth,the difference between the incidence of infection in the observation group(16.00%[4/25])and that in the control group(40.00%[10/25])was not significantly significant(P>0.05).Conclusion Oral smear of colostrum can effectively improve SIgA and LTF levels of premature infants,so that premature infants can obtain good nutritional support and immune protection,and thus reduce the incidence of infection.
