基于“伎巧出焉”探析肾与执行功能衰退的关系

摘　　要：执行功能是大脑的高级认知能力,通过对其他认知过程的控制和调节,产生协调有序的、具有目标指向性的行为,从而使认知系统能够以灵活优化的方式解决较为复杂的问题,与中医的“伎巧”高度相似。在认知功能衰退的过程中,与其他认知功能相比,执行功能衰退发生时间较早且影响最为显著,但现代医学对于执行功能衰退的病因及病理尚不明确,缺乏有效的治疗药物。中医学认为,肾为先天之本,藏精,主骨生髓。足少阴肾经通过经络循行,络膀胱,连督脉,最后通于脑。肾精亏虚,脑枯髓减,“伎巧”难出,渐成痴呆,引起执行功能衰退。Executive function is the advanced cognitive ability of the brain.By controlling and adjusting other cognitive processes,it produces coordinated and targeted behaviors,so that the cognitive system can solve more complicated problems in a flexible and optimized way,which is highly similar to the"skillful performance"of traditional Chinese medicine.In the process of cognitive function decline,compared with other cognitive functions,executive function decline occurs earlier and has the most significant impact,but the etiology and pathology of executive function decline in modern medicine are still unclear,and there is a lack of effective treatment drugs.In traditional Chinese medicine,it is believed that the Kidney is the foundation of congenital,which can store Essence,control bone and produce marrow.The Kidney meridian of foot Shaoyin,which circulates through the meridians,collaterals the bladder,connects the Du meridian and finally reaches the brain.Deficiency of Kidney Essence would lead to brain withering and marrow decreasing,hence it is difficult for skills to be applied.Then dementia gradually caused and the executive function declines.
