血清APE1-AAbs、CEACAM1联合检测在结直肠癌诊断中的效能

摘　　要：目的探究并分析血清脱嘌呤脱嘧啶核酸内切酶1自身抗体(Serum autoantibody to depurine and depyrimidine endonuclease 1)APE1-AAbs联合癌胚抗原相关细胞黏附分子1(Carcinoembryonic antigen associated cell adhesion molecule-1,CEACAM-1)在结直肠癌中的诊断效能。方法选取44例结直肠癌患者为观察组,同时选取70例结直肠良性肿瘤患者(结直肠息肉及结直肠平滑肌瘤)为对照组。收集并比较2组患者年龄、性别、合并慢性疾病、体重指数等情况,测定血清APE1-AAbs、CEACAM1水平。采用血清APE1-AAbs、CEACAM1及血清APE1-AAbs+CEACAM1联合等方法对所有受试者进行诊断,比较上述诊断方法的诊断价值。结果结直肠癌组血清APE1-AAbs、CEACAM1水平明显高于对照组,差异具有统计学意义(P<0.05)。单纯的血清APE1-AAbs、CEACAM1诊断结直肠癌敏感性、特异度无显著差异(P>0.05);平行联合诊断下,血清APE1-AAbs+CEACAM1诊断的敏感度、阴性预测值均显著高于系列联合诊断及单纯血清APE1-AAbs、CEACAM1诊断(P<0.05);而系列联合的特异度及阳性预测值显著高于平行联合及单纯血清APE1-AAbs、CEACAM1诊断(P<0.05);血清APE1-AAbs、CEACAM1、血清APE1-AAbs+CEACAM1平行联合诊断及系列联合诊断结直肠癌ROC曲线下面积分别为0.740、0.754、0.903、0.912,血清APE1-AAbs+CEACAM1系列联合诊断的ROC曲线下面积显著高于血清APE1-AAbs+CEACAM1平行联合诊断(P<0.05)。结论超血清APE1-AAbs+CEACAM1联合诊断较单纯血清APE1-AAbs、CEACAM1诊断更能有效提高结直肠癌诊断准确率,其中系列联合诊断具有更高的诊断效能,而平行联合具有更好的诊断敏感性。Objective To explore and analyze the diagnostic efficacy of serum autoantibody to purine and deprimidine endonuclease 1(APE1-AAbs)combined with carcinoembryonic antigen associated cell adhesion molecule-1(CEACAM-1)in colorectal cancer.Methods 44 patients with colorectal cancer were selected as the observation group,while 70 patients with benign colorectal tumors(colorectal polyps and colorectal leiomyomas)were selected as the control group.Collect and compare the age,gender,comorbidities with chronic diseases,and body mass index of two groups of patients,and measure the serum levels of APE1-AAbs and CEACAM1.All subjects were diagnosed using methods such as serum APE1-AAbs,CEACAM1,and a combination of serum APE1-AAbs+CEACAM1,and the diagnostic value of the above diagnostic methods was compared.Results The serum levels of APE1-AAbs and CEACAM1 in the colorectal cancer group were significantly higher than those in the control group,with statistical significance(P<0.05).There was no significant difference in sensitivity and specificity between pure serum APE1-AAbs and CEACAM1 in the diagnosis of colorectal cancer(P>0.05);Under parallel combination diagnosis,the sensitivity and negative predictive value of serum APE1-AAbs+CEACAM1 diagnosis were significantly higher than those of series combination diagnosis and simple serum APE1-AAbs and CEACAM1 diagnosis(P<0.05);The specificity and positive predictive value of series combination were significantly higher than those of parallel combination and simple serum APE1-AAbs,CEACAM1 diagnosis(P<0.05);The area under the ROC curve of serum APE1-AAbs,CEACAM1,serum APE1-AAbs+Conclusion The combined diagnosis of serum ape1-aabs+CEACAM1 can improve the diagnostic accuracy of colorectal cancer more effectively than that of serum ape1-aabs and CEACAM1 alone.The series combined diagnosis has higher diagnostic efficiency,while the parallel combination has better diagnostic sensitivity.
