摘  要:目的 探讨血清微小核糖核酸(miRNA)-155、miR-599水平对肝癌患者化疗后疾病转归的预测价值。方法 收集130例肝癌化疗患者的临床资料,按照疾病转归分为生存组、死亡组。比较2组患者临床资料差异,采用Cox比例风险回归模型分析疾病转归影响因素。建立受试者工作特征曲线(ROC),分析血清miR-155、miR-599水平对疾病转归的预测价值。结果 随访时间22~45个月,中位随访时间36.5个月,随访期间5例患者因肝癌外其他原因死亡,经排除后最终纳入125例。生存组与死亡组患者吸烟史、饮酒史、肿瘤直径、肝功能Child-Pugh分级、TNM分期、病理学类型、淋巴结转移、Edmonson病理分级、甲胎蛋白及血清miR-155、miR-599相对表达量比较,差异有统计学意义(P<0.05);多因素Cox模型分析显示,TNM分期、淋巴结转移、Edmonson病理分级、miR-155、miR-599为肝癌患者化疗后疾病转归影响因素(P<0.05)。ROC曲线显示,血清miR-155、miR-599水平检测对肝癌患者化疗后疾病转归预测的灵敏度、特异度及曲线下面积(AUC)较高,联合检测的预测效能更高,灵敏度为88.42%,特异度为86.67%,AUC为0.809。结论 血清miR-155、miR-599水平为肝癌患者化疗后疾病转归的影响因素。Objective To explore the predictive value of serum microRNA(miRNA)-155 and miR-599 levels on the disease outcome of patients with liver cancer after chemotherapy.Methods Clinical data of 130 patients with liver cancer treated with chemotherapy were collected and divided into survival group and death group according to disease outcome.The clinical data of the 2 groups were compared,and Cox proportional hazard regression model was used to analyze the influencing factors of disease outcome.Receiver operating characteristic curve(ROC)was established to analyze the predictive value of serum miR-155 and miR-599 levels on disease outcome.Results The follow-up time ranged from 22 to 45 months,with a median follow-up time of 36.5 months.During the follow-up period,5 patients died from other causes except liver cancer,and 125 cases were finally included after exclusion.There were significant differences in smoking history,drinking history,tumor diameter,child Pugh grade of liver function,stage of primary tumor-nodemetastasis(TNM),pathological type,lymph node metastasis,Edmonson pathological grade,alpha fetoprotein and the relative expression of serum miR-155 and miR-599 between the survival group and the death group(P<0.05).Multivariate Cox model analysis showed that TNM stage,lymph node metastasis,Edmonson pathological grade,miR-155 and miR-599 were the influencing factors of disease outcome in patients with liver cancer after chemotherapy(P<0.05).ROC curve showed that the detection of serum miR-155 and miR-599 levels had higher sensitivity,specificity and area under curve roc(AUC)for predicting the prognosis of patients with liver cancer after chemotherapy,and the prediction efficiency of combined detection was higher,with sensitivity of 88.42%,specificity of 86.67%and AUC of 0.809.Conclusion The levels of serum miR-155 and miR-599 are the influencing factors of disease outcome in patients with liver cancer after chemotherapy.
